0589561633 2013-04-04 Production Wells No Violations This inspection was conducted in response to complaint #3031 taken in 2011 by inspector Brett Farnsworth. The confidential complainant claims that this Historic Owner well is being produced and pumped by Robert Stryker. The location has had a new layer of gravel installed on the location and the lease road. There is a new flow line installed to the 100 barrel production tank that appears to be newly installed next to an older rusted 100 barrel production tank. This new tank is in violation of the tank spacing requirements as it is nearly touching the old tank. It is also in violation of the well spacing requirements as it is only 18 feet from the wellhead. The well shows no change of ownership, production and does not have any identification onsite of any kind. The tank battery is also without an SPCC dike. The owner/producer of the well needs to be identified and proper action to register the well and bring it into compliance is needed.
0590037672 2015-02-13 Production Wells No Violations On the date of inspection I found that production equipment consists of two 100bbl tanks, a 50bbl tank on its side in the weeds, and a conical style separator. The well is equipped with a pumping unit, rods and tubing. The only production reported was in 2013. Identification was in place on the electric pole. The well is currently shut in and not connected to any tanks. Weather conditions are mid-teens with one inch of snow on the ground.
0858211470 2018-05-24 Production Wells No Violations On the date of my inspection I found this well equipped with a walking beam pumping unit, rods, and 2.375 inch tubing. The casing size could not be determined as it is buried. Production equipment includes two 100 barrel steel tanks and a 50 barrel poly tank. Production records are current and the well has not produced since 2014. Plugging permit expires 9/20/19. Voicemail left for Bob Stryker in regards to plugging. Bob Stryker returned my call and plans on plugging well this summer.
1368223552 2014-07-25 Production Wells No Violations Purpose of this inspection was to verify that the Operator has taken steps to remedy compliance issues on site. Scald area downslope of the wellhead has been mulched. Wellhead has been disconnected from the conical style separator and the black plastic line from the seperator has been pulled up out of the ground. The well has been shut in and is no longer connected to the production tanks. Operator is planning on applying for a plugging permit and plugging the well.
1377811266 2013-08-28 Production Wells No Violations Check on complaint #3031. Checked the location. No change in setup or wellsite identification. Tried to contact pumper assumed to be using the well with no response at this time. Will try again in near future.
1403211066 2014-06-17 Production Wells Violations Noted Onsite with supervisor Brent Bear. Purpose of this inspection was to follow up on complaint #3031. Robert Striker is the contact person for this well, 330-227-3568. Annual production needs to be reported. A sign has been posted on the site indicating the name of the well as Cooper #1. Inspector contacted Mr. Striker to have him add the permit number and the county to the Identification tag. Wellsite does not have a containment dike. A pumping unit and 2 100bbl tanks were onsite. One of these tanks is not plumbed to the well. The other tank has a conical style seperator attached that is plumbed directly to the wellhead. A 1.5 black plastic pipe comes off of the back side of the seperator and is buried underground. No brine tank is present. A scorched area starting approximately 50 south of the wellhead measures approximately 30x15 with a scorched channel continuing south below that area for another 60. Chlorides in the 30x15 scorched area were field tested and found to be 1432 ppm. Brine must be collected and disposed of properly and the affected area must be remediated within 30 days. A steel line is protruding from the ground off of the southeast of the well area with evidence of a healing scorched area.

1890343533 2016-08-12 Production Wells No Violations On the date of my inspection I found two 100 barrel tanks, a cylindrical separator attached to one of the tanks, and a 50 barrel tank on its side in adjacent to the other tanks. The well is equipped with a pumping unit, rods, 2.375 inch tubing and 4.5 inch casing, and is shut in. Proper identification was found at the power pole adjacent to the pumping unit. Production records are current, zero production in 2015.
