0087483256 2016-12-27 Final Restoration No Violations Today I am onsite to perform a final restoration inspection for this permit number. In accordance with ORC 1509.072(B), all oil and gas related equipment and debris has been removed from the area, it has been returned to grade, and is re-vegetated. This site passes for final restoration.
0252269356 2010-11-01 Plug / Plug Back No Violations Inspection revealed that the cement has dropped downhole approximately 20 . I contacted Louis Beucler and advised that he needed to fill the casing with cement to within 3 of the surface before cutting off the casing and welding the plate on top of the casing. No violations at this time.
1291937718 2010-10-27 Plug / Plug Back No Violations Inspection revealed that the service rig is pulling the tubing and then going to rig up to pull the 6 5/8 casing. No violations at this time.
1410547969 2010-10-28 Plug / Plug Back No Violations On site to witness service rig trying to pull the 6 5/8 casing. Rig tried to pull the casing but the casing wouldnt move. They attemted to do a blind back-off and the casing backed-off approximately 4 downhole. I advised to screw the casing back to and try to pull it again. The casing didnt move again soI advised that could leave the casing in the well. I advised that they could run a siphon string into the well and plug the well with a grout mix.
