0167998126 2016-07-18 Plug / Plug Back No Violations I went to Cambridge Office and got coal maps for 081-6-1608. I went over the maps with Mike Kosek. The following per Mike was stated that the well is in room of the mine. He printed some copies for me from plotter printer.
0227554303 2016-10-25 Plug / Plug Back No Violations I am on site with Louis Beucler with Beucler Brothers Inc to test the water in the lined pit used for the first phase of plugging this well. The pit has filled with rain water/runoff from recent storms. A sample was taken and using Quantabs the chloride level is approximately 237 parts per million. Louis indicated he would have a brine truck come in and empty the pit as soon as the ground was hard enough to drive on.

0801786220 2016-08-10 Plug / Plug Back No Violations I called Louis Beucler on well that was being worked on to be plugged. I stated that the minimum that can be done is to isolate the well from the mine floor and isolate the Mine ceiling to the surface to protect fresh water at 35-

-61608. Louis called him and he would like for Bill to get with Gary Smith of Kilbarger Drilling in Logan and come up with plan to drill out the well for plugging purposes.

1471016962 2016-08-02 Plug / Plug Back No Violations I met with Bill Mitton and had a conference call with Louis Beulcler on well 081--round tubing. Bill will have the tool made up and it will have Tungsten Carbide installed on the mill. There were other plans also discussed shut as water situation and a second plan in case the taper mill does not work.
1478201490 2016-11-03 Plug / Plug Back No Violations Today I am onsite to verify the closure of a lined pit used during the attempted plugging operations for this well. Beucler Brothers Inc. have emptied the pit and removed the liner. The pit area has been restored to grade using the spoils from the digging of the pit initially.
1479490671 2016-11-18 Plug / Plug Back No Violations Today I am onsite with Inspector Pat Shreve to observe Appalachian Well Services running a camera down the well. Beucler Brother Inc is also onsite to observe the operation. Water with a thin oil layer was at approximately 8 to 9 feet below ground surface. Appalachian attempted to run the camera down the well multiple times. The high turbidity in the well made for very poor visibility. A possible pipe could be seen at approximately 50.6 feet. Sediment had built up in the pipe from 51.6 to 53 feet, making the identification of anything else impossible. Beucler Brothers will have to come up with a different plan to move forward with the plugging of this well.

1507047514 2017-10-03 Administrative Inspection No Violations Today I met with Louis Beucler along with Brent Bear, Mike Moreland, and Pat Shreve in New Philadelphia regarding the plugging of this well. We discussed the path forward, which included Louis submitting a plugging plan. He indicated that he would contact a few companies with top hole rigs about working on the well and would submit a plan within approximately 3 weeks. It was determined that he should attempt to reach depths of approximately 50 feet below the known mine in the area, which is approximately 100 feet below the ground surface.
