1395430380 2014-03-13 Exempt Well Pre Transfer No Violations I was on location to respond to an emergency gas leak situation. The Stratton Fire Department was called around 7:30 a.m. to the area of the entrance to 84 Lumber and worked to evacuate the residents to the Stratton Village Building. I was contacted at 8:30am by Jefferson 911. I arrived on located at 9:30am and Columbia Gas shut in the well by closing the valve on the flow line on the well. Well appears to be old Berea well. No API# or records of well. No readings on PHD6 gas meter. Before I arrived there was an evacuation of about 30 families at the bottom of state Route 152. State Route 152 and the nearby Norfolk Southern railroad line were ordered closed. Jefferson County sheriffs deputies and troopers from the Ohio State Highway Patrol blocked roads to prevent vehicles from traveling into the area. We assessed the situation and talked with owner/user of the well Ruth Fecik, 209 Main St. Empire, OH. She is elderly and her contact is her son John Fecik 740-381-9089. Says well was present when she bought the house 67 years ago Well is contained in a Rubbermaid shed. Currently 154 and the rail line are open again. People have been let back into their homes after the fire department goes through to check for gas. At 12:45pm the well again purged and empire fire department recorded a reading of 125% LEL on their gas meters on the road. The well appears to not be full stable at this time. Tried to contact a Samuel Shevelo, only known person to work on the well years ago, without any success. Moore Well Services were contacted and have corrected the issues at the well. They have removed the condensate line that was leaking from the well. They cut it off and installed a bull plug. The valves were tightened and are functioning at this time. We installed a pressure gauge and well had 62psi on it. The landowner has found the deed to the house with it showing ownership of the well and I have given them the Exempt Domestic Well Forms. Moore Well recommended to the land owner
1492544418 2017-04-18 Production Wells No Violations Previous inspection reports indicate that this is a domestic well and is enclosed in a locked shed. Shed was still locked. A complaint came in with concerns over bubbling in the Ohio river at mile marker 29 on SR 7. There is a Columbia gas transmission line under the river in this general location but no bubbling was witnessed. Information was given to Scott King, emergency response supervisor. This well was checked as it was the closest well to the complaint location.
1775194840 2015-03-31 Production Wells No Violations On the date of inspection I attempted to check the status of the well. The landowner constructed two small sheds around the well and equipment with pad locks. I was unable to find the landowner at the time of inspection to gain access. The previous inspection of the well had been due to an emergency gas leak.
2092030563 2016-08-12 Production Wells No Violations Status is the same as the last inspection: On the date of my inspection I attempted to check the status of the well. The landowner constructed two small sheds around the well equipped with pad locks. I was unable to find the landowner at the time of inspection to gain access.
