0009641951 2013-03-27 Orphan No Violations An inspection conducted with Oil & Gas Inspector, Mark McCombs, revealed a wellbore that measured 14 in diameter with no visible casing at the surface. The well was situated approximately 500 away from a residential dwelling and located on the edge of an agricultural field. There was no discharge of fluid or natural gas at the surface. At time of the inspection, the well did not contain coordinates in RBDMS. Central Office will be notified of this deficiency. A GPS reading was obtained for the access entry point.

1026008629 2015-02-18 Production Wells No Violations Well is uncased open hole. Submitted to the orphan well program 2013. Per Gene Chinis request well needs updated reports. This date per the landowner a steel plate covers the well. Eric Daugstrup and myself walked to well area using previous GPS readings. Ground was snow covered and unable to locate well at this time. Will contact landowner for more information.
1110161430 2013-05-29 Orphan No Violations A follow-up inspection conducted with Orphan Well Program Supervisor, Gene Chini, revealed no change to the well status since the last inspection. As previously noted, the wellbore diameter measured 14 and did not contain any visible casing at the surface. A depth counter equipped with a braided line and a 2 oz sinker was utilized for this inspection to gauge the wellbore depth. The total depth was measured at 180 and the material that was encountered resembled a soft clay-like material. There was no observed leakage of gas or fluids from the well. Two steel plates have since been placed over the well for safety. Prior to this inspection, the well location had been missing coordinates in RBDMS and a request to review this location was sent to Central Office.
1364235304 2013-03-25 Orphan No Violations I received a call today from Rick Cannon. He stated he had found a hole in his yard that he thought was an old Clinton well. I went to the property and met with Mrs. Kris Cannon. She showed me where the well was located. I was able to locate it on the Knox County Monroe township Ohio Fuel Maps and match it to the API well number in the RBDMS data base. The hole is a 14 in. + open hole. I returned later with Tom Benko and ran a fish line in the hole to get a depth. There was a seat at 60 feet and it bottomed out at 160 feet.
1422542051 2015-01-23 Administrative Inspection No Violations I received a phone call from land owner Rich Cannon.(740) 398-4121. He wanted to know the status of this well on his property. This well was turned into the Orphan well program 2 years ago. He is concerned for children. I will check into status for him. 01-28-15 I found Brent Bears paper copy orphan file that was submitted to the Orphan Well program on 08-21-13.

1426597248 2015-03-13 Production Wells No Violations Well is listed as production in RBDMS. This well is not producing and has made application to the Orphan well program. Well file update. Land owner has installed a flat steel plate over open well bore. This allows him to mow over this area. This date I did not try to pry plate off and look into well. No visible or audible fluid or gas leakage. Per Brent Bears Inspection of 05-29-13 the well is a 14 inch open hole with a measured depth of 180 feet. Original orphan file was turned into the New Philadelphia Office on 08-21-13. I took 6 additional photos.
1453919047 2016-01-27 Administrative Inspection No Violations This well is in the orphan well program. I inspected this well with Brent Bear three years ago and again today. Yesterday 1/26/2016 Mrs. Cannon called me and was wondering when this open hole issue was going to be resolved. Her concern is not only for her small children but the neighbors children as well when they come over to play. They have put a this steel plate over the well open bore to help prevent a child from falling in to the well. The well bore is 14 to 16 inches in diameter open to the 8 inch surface seat and beyond. With all the existing circumstances, conditions and possible consequences this well should be looked at as an emergency issue and taken care of as soon as feasibly possible.
1456405393 2016-02-23 Orphan No Violations Well status is orphan well pending. I updated well file at the request of Orphan well Program Supervisor Gene Chini. I was accompanied to the site by DOGRM Knox County Inspector Mark McCombs. I took a GPS reading of the water well N40.42448, W082.37752. We took the steel plates off the top of the wellbore hole. The wellbore is not leaking fluids or gas. A depth counter reel with 20# test monofilament line and a 2 ounce sinker was utilized to gauge the wellbore depth. The depth was 133. Fluid was encountered at approximately 75. The wellbore is 14 in diameter with no visible casing. The steel plates were covered back over the top of the wellbore and pinned in place. Pins or steel brackets were pushed into the ground at the four corners to help secure the steel plates. I took eleven additional photos.
1508786470 2017-10-17 Orphan No Violations Well Inspection with DOGRM Senior Health Physicist Rob Leidy. Met the landowner Mr. Cannon. Well site has changed from a mowed yard to an extension of the existing crop field. Well is surrounded by soy beans.
