1369061516 2013-05-20 Production Wells Violations Noted I inspected this well today with Brandon Mitchell in response to a well fire. Found three 100 barrel tanks that had been disconnected, holes cut into the sides, there was wood put inside one of the tanks and ignited to burn out oil residue left in the tanks. There were 2 small pits dug into the dirt and crude oil was drained into them to be burnt for disposal (OAC 9-1-07 Well operation causing pollution and contamination ORC 22(A)). There is no identification at the well or tank battery (OAC 9-9-05(A) 10 Failure to legibly identify well ORC 23). The pump jack has been removed. Production line has been cut up and removed. The well is not capable of production.

Mr. Crandell stated that he had 2 individuals working cleaning up the property and the removal of the well equipment was part of that project. I talked with the individuals about what had happened they stated that they had drained part of the oil into 55 gallon drums, but that was taking too long so they dug 2 pits and drained the oil into them to burn (OAC NONE (A) Unlawful method of storage or disposal of brine or other wastes ORC 22(C)) (OAC NONE (E) Use of pit of dike for ultimate disposal of brine or other wastes ORC 22[C]. They then put wood into the tank to burn out the residue and fire the pits after cutting holes into the side of the tanks. They stated that the fire was hard to get started but once it started it went out of control with hot flaming oil blowing out the drain hole in the tank into the stream beside the tank set and into the wooded area burning the trees and the oil in the pits and the stream that was dry at the time (OAC 9-9-02 General Safety ORC 23). Remedy sought is clean up
