1288522753 2012-03-01 Production Wells Violations Noted The Division received a complaint alleging pollution and contamination. An inspection of the well and related tank battery locations revealed no pollution and contamination. Verified and explained with complainant representatives that alleged locations were iron scales/bacteria not crude oil. Verified no connection for annular disposal. While conducting an inspection of tank battery location consisting of (2) 100 bbl tanks, (1) 50 bbl tank and separator, Mr. Roman Mast introduced himself as the well owner. Mr. Mast states that he purchased the property and well in May 2011. Mr. Mast resides on neighboring property (well on complainants property & tanks on Mast property). Mr. Mast receives house gas from this well. Discussed well ownership requirement along with identification and provided Mr. Mast with necessary paperwork to file as registered well owner. Mr. Mast was unaware of registration and agreed to resolve. Mr. Mast will consult with Ms. Erica Freeman at Central Office.
1374499978 2013-07-22 Production Wells No Violations I inspected this well today. Ownership has been transferred to Helms Oil.
1522259211 2018-03-27 Production Wells No Violations I received a call on an oil spill on this well. When I arrived Mr. Helms was in the process of putting down saw dust to absorb the oil that was on the ground. Approximately 3 to 5 gallons of oil had been released through a blead off line that someone had tampered with. Mr. Helms also dug a small ditch on the downside of the spill and had placed booms to catch any run off from the rain. Mr. Helms is bringing in equipment to remove any contaminated soil and continue on going clean up of any oil residue. I suggested to Mr. Helms that it may be in his best interest that a fence be placed around the pumping unit do to the fact that there were small children playing in the area from a home in close proximity of this well.
1522433850 2018-03-30 Production Wells No Violations The area is free of oil sheen on the ground the saw dust is free as well. The booms in the ditch have collected some residual oil and will need to be removed and replaced. I called Mr. Helms and left a message. I will follow up on 4/2/2018.
1615767206 2018-03-29 Production Wells No Violations The area around the pump jack was free of any sheen of oil today. It was raining during this visit and the area of containment was still in satisfactory condition. I will follow up again on 3/30/2018 to monitor the progress.

1846724138 2018-04-18 Production Wells No Violations The area of the oil release has been cleaned up and is free of oil and any sheen in the area. The booms that are there are clean with the exception of some dirt splashed on them from the excessive rains. There were no other issues to note with this well.
1920472002 2012-04-02 Administrative Inspection No Violations The alleged well owner, Mr. Roman Mast, notified me by telephone today at 9:26AM to state that he spoke with Ms. Erica Freeman regarding well ownership. Mr. Mast further stated that he will most likely transfer this well to his pumper, a registered well owner with the State of Ohio.
