Note: 8.8 units is equal 6562 ppm (mg/L) Cl- (chloride).
0939527812 2014-09-29 Orphan No Violations This inspection was conducted as a status check. Barry Bowers moved in rig during inspection and prepared location for drilling operations.
1145741563 2014-10-09 Orphan No Violations Orphan well plug day 8. I arrived at 10:30 am. Barry Bowers rig crew of 5. Drill pipe and power swivel has been removed from the location. Crew ran 1098 feet of 1 1/2 tubing into well to plug. Barry and his son arrived at 12:00 noon. They pumped and achieved circulation after performing cementing calculations. They pumped approximately 100 sacks of class A cement weighing 15.2# when the chain feeder broke on the bulk cement truck. Cement operations shut down and rig crew pulled 9 joints of 1 1/2. They flushed the tubing with fresh water and made temporary repairs. Crew then ran 2 joints of 1 1/2 tubing back in the well and tagged the cement top. Well was then cemented with the rest of the cement from bulk truck. Cement weight 15.3#. Target depth for first plug is 444 feet. Crew to pull rest of 1 1/2 out of the hole and finish the top cement plug tomorrow. I left the location at 4:30 pm.
1155749086 2014-10-08 Orphan No Violations Orphan well clean out day 7. I arrived at 9:00 am. I visually checked the 6 inch flow line end at the steel collection tank. No fluid movement, no visual or audible gurgling. Checked also with gas meter which read zero. Barry Bowers plug rig crew of 4 started drilling and reached a depth of approximately 1000 feet. Division records show that there should have been 908 feet of 4 1/2 casing left in the hole. We double checked the Division records and reports for this well and Permit #3430 Besst 2 which was a lost hole. Permit 3430 was plugged and abandoned and the rig skidded to P# 3569 Besst 2A. Drilling commenced thru thick plugging clay to 1115 feet and encountered metal. Drilled for .5 hour and no gain. Decision made to stop there circulate the hole good and trip out to prepare to plug the well. I had ran this past Gene Chini the day before about stopping on top of the 4 1/2 rip at 908 feet. There is no fluid or gas flow at this time and Gene approved stopping on the 4 1/2 rip. They plan on cementing tomorrow. I left at 2:00pm. I met their 2 man crew coming to the location in their cement bulk truck.
1345633890 2012-01-17 Orphan No Violations The Division received a landowner complaint alleging that a Jadoil Inc. plugged and abandoned well had been leaking for last 10-15 years and progressively got worse this past summer. An inspection revealed a large rock measuring approx 5x 5, in pasture field, placed over top of the wellbore with observed bubbling and fluid discharge around the outside of rock. A gas detector was utilized, but were unable to document gas show. The immediate area lacked vegetation and resembled the shape of a circle measuring approx 25 in diameter. High Range Quantabs placed in the fluid showed 6 units, which equals 1716 ppm (mg/L) Cl-. There was no observed oil discharge from wellbore at this time. An intermittent stream was located approx 200 away to the North. There were no overhead obstructions or lines at the well location. Property is floodplain to the East. The complainant confirmed the placement of rock over wellbore with tractor to help prevent flow. He agreed to remove the rock to allow further evaluation of wellbore. A GPS reading was obtained and confirmed well location with Central Office.
1402320036 2014-05-22 Orphan No Violations Well is on emergency orphan well plug list. Inspector Ben Harpster and myself met the landowner Roger Besst and plug contractor Barry Bowers on location to view and go over plugging requirements. Barry will be submitting a bid proposal for this well.
1403097709 2014-05-22 Orphan No Violations At the time of the inspection I was on location do discuss the logistics of plugging the well for the Orphan Well Program. Also on location was ODOGRM Inspector Jeff Fry, Plugging Contractor Barry Bowers of Huffman-Bowers Well Service, and landowner Raymond Besst. The well has approximately 10 casing and was under a couple inches of water in a muddy area of a cow pasture.
1411397280 2014-09-22 Orphan No Violations Arrived at well site at 8:45 am.. I informed the landowner Roger Besst that Barry Bowers the plugging contractor would be moving equipment in the end of this week. Roger and I walked to the well. Area around the well is dry and the 10.75 casing is still open. Fluid level is down approximately 1 foot. The top of the fluid is bubbling some and looks foamy. Roger plans on moving wagons out of the way, and cattle are in this area behind the barn. This date at 9:24 am I received a call from Barry Bowers who plans on moving some equipment in this Thursday around 11:00 am. He will confirm with me on Wednesday with more details. Barry also wanted to know the conditions around the well head. I called Roger Besst at 9:29 am. And advised him of the move in date.
1412098165 2014-09-30 Orphan No Violations Contractor Barry Bowers moving in with plugging equipment and rigging up on day two. Fluid level inside casing is approximately 1 foot below ground level and still bubbling gas. Ran 4 tools ( jars on rope socket) on wireline to a depth of approximately 89 feet and tools set down. Wooden terra mat pads put down around well head. Well head control has been installed on the 10.75 casing. Steel collection tank set for pit. Single 300 Bbl. Frac tank set for well control with saltwater. New 9 5/8 bit with 1 joint drill collar and joints of 3 1/2 inch drill pipe used. Drilled with power swivel to approximately 115 feet and shut down for the day. I arrived at 10:50 am and left at 5:00 pm.Landowner Roger Besst was present. I also met Raymond Besst Rogers father. Landowner still has dairy cattle in this area.
1412178773 2014-10-01 Orphan No Violations Orphan well cleanout and plug, day 3. I arrived at 10:30am. Barry Bowers and 5 man crew running power swivel at 150 depth. I left location at 4:00pm. Hole depth approximately 418 feet. Barry decided to stop here just above the concrete precast plug on the 8 inch casing seat. He will start up tomorrow morning. No problems so far on cleanout. Cattle are still in this area.
1412345227 2014-10-02 Orphan No Violations Orphan well clean out day 4. I arrived at 10:30 am. Barry Bowers rig crew of five. They had to go and get a pallet of gel to increase circulation materials. Bit was sticking and they were losing hole. Gel on site at 11:00 am. Steel drill tub had been emptied except for solids. They power mixed gel in steel tub and started drilling at 2:00 pm. Drilling was slow, with good circulation. Barry stopped drilling at approximately 444 feet on top of cement tapered plug where hole reduces to 8 inch diameter. They plan to start back up next Monday. I left the location at 5:30 pm.
1412615738 2014-10-06 Orphan No Violations Orphan well clean out day 5 .I arrived at 8:00 am. Drill string is in the hole to approximately 444 feet. Division plug records of 1985 show a precast cement plug set at 444 feet. The hole is loaded with drilling gel. I could hear gurgling and could see a small amount of fluid spitting out flow line into steel tub. Barrys crew of five started drilling on precast at 9:00am. They made approximately five feet and it acted like they were thru the precast plug. They will circulate twenty minutes and trip out, and change to a 7 7/8 bit. They will then trip in and resume drilling or clean out. I left at 10:30 am.
1412875589 2014-10-07 Orphan No Violations Orphan well clean out day 6. I arrived at 12:30 pm. Barry Bowers plug rig with a crew of six. Drilling at 500 feet on metal. Grab sample( return fluid) has metal flakes in it.. Drilled 3.5 hours and gained 1 foot. Fluid level in steel tank down 3 inches. At 1:50 pm bit broke thru and dropped approximately 60 feet before drilling hard again. Drilling continues this way chasing metal object down hole. It is possible that the original plug crew in 1985 may have thrown the bottom 4 1/2 rip collar of the 4 1/2 casing down the hole. Clean hole out to 840 feet. Rig crew will circulate the hole and stop for the day. Metal object appears to have been drilled up. I left the location at 4:00pm.
1412946005 2014-10-10 Plug / Plug Back No Violations Orphan well plug job day 9. I arrived at 10:00 am. Barry Bowers plug rig crew of 5. Well bore has no visual or audible fluid or gas flow. Four inch tools run in well and tagged the cement top at 510 feet. Rig crew ran 1 1/2 inch tubing to this depth and circulation achieved. Crew started pumping class A cement 15.5# until cement circulated into steel tank. Pulled 1 1/2 inch tubing. Well bore was full of cement to approximately 3 feet below ground level. Cement slowly dropped to approximately twelve feet. Rig crew started clean up and tear down. They will be back on Monday which is a State Holiday ( Columbus Day). I will follow up on Tuesday. I left location at 2:30 pm.
1413393355 2014-10-14 Orphan No Violations Orphan well clean out and plug follow up. I arrived at 10:30 am. Well head still has flange bolted on it and sticking above ground level. Well has been topped off with cement and cement is approximately 30 inches below ground level. No visible or audible leaking. I held the gas meter in this enclosed space and showed zero readings. This plug job corrected any leaking conditions that existed before clean out and plug. Steel tank has been emptied of fluids and is 1/3 full of solids. No reclamation yet or soils removed. Plug rig is gone. Equipment still on location outside the cattle lot is the, cement pump truck, rig circulation pump,backhoe,generator, and wood skids. I left the location at 11:30 am.
1413831438 2014-10-20 Orphan No Violations Barry Bowers notified me that he would cut off the well casing today. I arrived at the well site accompanied by Inspector John Williams at 10:30am. Well head had been dug around to a depth of approximately 5 feet. Well was cut off with power saw at 4 feet from ground level. Casing was full of cement. A steel identification plate with the letters ODNR and the permit number were welded to a flat plate attached to a short piece of 8 inch casing. This was inserted into the inside of the 10 inch casing after cement chipped out. Operator removed the soil from around the wellhead and will take to the landfill. Dozer was on site leveling out soil. Six photos were taken. GPS measurements were taken. We left the location at 11:30 am.
1414590606 2014-10-28 Orphan No Violations Orphan well is plugged and abandoned. I received a call from Barry Bowers the plug contractor on Friday 10-24-14 saying reclamation would be finished on that Friday. Today I checked well location. All plugging equipment has been removed. The excavation around the well head has been filled in and the area final graded and seeded. Barry Bowers went above and beyond his contract and leveled a high spot for the landowner and filled in a couple of low spots. He brought in a dump truck load of limestone to put on the drive way. Landowner requested he dump it in a single pile and he will add to drive as needed. Stone was dumped behind the horse barn. I talked to Roger Besst the landowner at the site. Roger is very happy with the way everything turned out. Cows are still in this area. I told Roger I would be back in about a month and visit the site for final restoration. He has my office and cell phone number if any questions.
1417193182 2014-11-26 Orphan No Violations Well was P&A on 10-10-14 under the orphan well program. Well site is firm and has no depression. Cattle are still in this area and the well site and surrounding area is trampled on by the cattle. Land owner has filled in a couple of low spots in the driveway with the lime stone that plugging operator left. All equipment has been removed. Four photos were taken. Bills and paperwork have not been submitted by plug contractor to the office. This will trigger my Final restoration inspection after review of paperwork and approval of the site with the land owner.
1423844658 2015-02-13 Final Restoration No Violations Well was P&A on 10-10-14 under the orphan well program. Checked site for final restoration at the request of Michele Miller. Site is frozen with some snow cover. Site passes final restoration requirements. 2 photos taken. I was accompanied by Will Ziegler.
1427389098 2015-03-26 Final Restoration No Violations Well was plugged and abandoned through the Orphan Well Program on 10-10-14. Landowner Roger Besst called and said his cattle when let out of the barn are going to the old well site and drinking the water. Roger said cattle never bothered this spot after the well was plugged, until the recent rains and the snow melt. This date I observed cattle drinking from their own muddy hoof prints on the East edge of the old well site. I collected a surface sample from a hoof print in this area, and a sample from a hoof print located North of the well site. Both areas are muddy and cow manure is visible on the surface. Both samples were tested with a low range quantab. The first sample from the East edge had a reading of 4.6 or 194 ppm chloride. The second sample North had a reading of zero or no chlorides. I explained to Mr. Besst that the first reading is below the Ohio EPA secondary water standards of 250 ppm. The recent rains may have flushed chlorides from a spot of soil that was not removed when the well was plugged and the restoration performed. Mr. Besst has no salt or mineral blocks set out for the cattle. I informed him we will revisit the site later in the spring. If any thing else changes let the Division know. I informed him the only other thing he could do now is to fence of this area to keep the cattle away from it until things dry up and start to grow.
1429109957 2012-03-26 Orphan No Violations A follow-up inspection revealed that the complainant removed the rock covering the wellbore. Fluid level was observed at the surface of wellbore with constant bubbling however, were unable to detect with gas meter. A shovel handle was placed inside casing and obstruction was encountered. Although casing was covered with fluid, an attempt to measure diameter confirmed approx 10 steel conductor. 2 sections of steel approx 4 x 6 length were uncovered near wellbore under mud and possibly cutting the 10 below grade after P/A.

A sample of the fluid obtained from wellbore was tested with High Range Quantabs and showed max units, which exceeds >6000 ppm(mg/L) Cl-. No oil discharge was observed at this time. Records indicate Trempeauleau (dry hole) p/b to Oriskany then p/a. Plug reports indicate all 445 of 8.63, and 1690 of 7 csg and 908 of 4.5 csg (95 sks) were retrieved. No record of total 4.5 ran, but had plugged back to depth of 1348 before eventual p/a with perfs @ 1224-1230, 1200-1208. Approx 237 of 10 conductor was left in wellbore. Clinton formation at 2065-2100 and behind 7 csgw/15 sks. Cattle in pasture field so precaution should be taken with earthen pits and equipment. Recommend temporary fencing to eliminate potential accidents. Minimal restoration required. Evaluate below perfs at 1300 and to Clinton. Possible reopen to Trempealeau.
