1083769847 2004-05-05 Production Wells No Violations This was a follow up inspection at request of Columbus Office. This well was not found during the inspection of the plowed farm field. Inspection was done w/ Jeff Fry.

1902475535 2015-01-22 Production Wells No Violations This well is listed as a historic owner, the inspection was requested by the land owner, the inspection reveled that the well is not being produced and there is no production equipment associated with this well, there is a 3 feet section of pipe sticking up in the corn field that is full of water and is bubbling gas out continually. There is also a sheen of oil on the top of the water that is surrounding the pipe there is an area of 20 square feet around the pipe that is being impacted by this well. It appears that this well may have been plugged at one time and is leaking but there are no supporting documents to support this claim. The well is also located in a flood plain close to the river and could cause potential problems with stream contamination as well. I have notified Inspector Jeffery Fry with the orphan well program and will follow up with him.
