0027795829 2017-10-17 Orphan No Violations Well Inspection with DOGRM Senior Health Physicist Rob Leidy. Well continues to leak fresh water to the south east in a saturated area of approximately 80 feet by 12 feet on the tail end before disappearing into the ground. This well is bubbling H2S gas and I recorded a peak 1.6 PPM reading on my Tango Gas meter. The reading came from the inside of the 8-inch diameter casing. We met the adjoining landowner Kim Waggoner and requested access thru his property to this well. Mr. Waggoner agreed and showed us the access he would like used off of State Route 13. This will require the replacement of a drainage culvert to obtain access. We left a new Right of Entry form for him to sign and return. This access will make access to the wellsite easier than crossing a deep road ditch on State Route 13 and up over an old railroad bed. The well is land locked on the other side by the Kokosing River which is a state scenic river. I took 8 additional photos and a movie of the leaking well.
0332078605 2013-06-12 Orphan No Violations A follow-up inspection revealed that the severely rotted 8.5 outside diameter casing continues to discharge clear fluid with no elevated chloride, and gas detection with meter. The gas meter utilized for this inspection was equipped with an automatic pump attached to a probe with a filter. The meter was placed next to the wellbore and showed the following: LEL = 7 and CH4 = 0.25.

1198085645 2007-12-11 Orphan No Violations Old Gas well. 8 casing sticking up approximately 2. No other casing visible. 8 has corroded at ground level and natural gas is bubbling through water. Water level inside casing is flowing out and into field. Area is approximately 60 by 30 wet area with odor of lime water or hard water. 11 photos taken and I&O packet started. Well reported to the Division by Richard Stallard of the Owl Creek Conservancy group. 202 South Edgewood Rd. Mt. Vernon 740/ 392-2901. They are in process of purchasing property. Surveyor John Dewalter (cell 740/627-0650) found well. Plug application / right of entry sent to Richard StallArd who will take it to current landowner Daniel Galbraith.
1204111315 2014-05-19 Orphan No Violations Well is on the Orphan well program to be plugged. This date update file with GPS reading and six additional photos. Well is still leaking fluid and gas. No visible crude oil. Well is leaking water with gas bubbling thru the water. Slight odor of H2S. I parked off of State Route 13 onto the old Railroad bed. When leaving I was approached by Mr. Kim Waggoner who is the adjoining property owner who I was parked on. He wanted to know who I was and what I was doing. I explained to him who I was and that I was updating the well file. We exchanged names and phone information. He said he used to farm the Galbraith property and they are not getting along as neighbors. He said that he would not allow access to the well site thru his property. He said years ago the State Highway Department closed the only access to that field off of St Rt 13 when they put up guard rails. The field is bordered by the Kokosing River to the west. Mr. Waggoner is tired of running people off his property who are trying to access the Galbraith property. I apologized for parking on him and told him I didnt know. He said that now that I know there would be no excuse in the future for entering. His property is not posted. There is a sign posted on the Galbraith property of private property Riparian Area. This well is going to be difficult to access.
1354888812 2012-12-04 Orphan No Violations A follow-up inspection was conducted with Mr. Jeff Fry to evaluate the status of this orphan well. During the inspection, it was noted that the well exhibited similar conditions that were previously documented by Mr. Fry in 2007. The well consisted of only 8 casing with no wellhead at approx 2 above ground surface. The 8 casing was rotted in half and the bottom section of casing that remained in the ground contained a hole on the side of casing, which allowed for a fluid discharge. The fluid was clear and contained an odor that resembled Lime/hard water. The fluid from this well flowed to an area measuring approx 60 x 30 and contained iron scales on the surface of water. Consistent gas bubbling was also observed inside the 8 casing, but no detection was found with the gas meter. There were no overhead obstructions around the well. An electric tower and power lines suspended in the air (>50) were also observed around the well location. The base of the tower was located approx 50 away from the well and power lines do not hang directly over the well. Note: The I/O Referral case will need to be located to review the Right of Entry (ROE) Forms for the following: First, an ROE could be required if the well falls within the Power Line Easement. Secondly, the access to this well is limited and will likely require entry from a neighboring property. Therefore, an ROE would be required from this property owner.
1403099354 2014-05-22 Orphan No Violations At the time of the inspection I was on location with ODOGRM Inspector Jeff Fry to discuss the logistics of plugging the well for the Orphan Well Program. The status of the well is unchanged from previous inspections and the SEM score does not need adjusted.
