0009009248 2017-06-02 Production Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-459, issued on October 15, 2014 for failure to maintain liability insurance coverage in accordance with Ohio Revised Code (ORC) 1509.07. As part of the order, Future Productions, Inc. (Owner) was directed to suspend all oil and gas operations in the State of Ohio. At the time of this inspection, the Owner had failed to correct the deficiency. I observed the well to be in active production. I also noted that no production reports had been filed since 2012. I contacted Brett Huntsman on 06/05/2017 and advised that the Owner was in violation of the referenced Chiefs Order and must suspend operations pending the renewal of liability insurance. We agreed the Owner must resolve wihtin two weeks, or by 06/16/2017.

The well was in active production as evidenced by an active chart in the chart recorder which was current as of the day of this inspection as well as open and active flow-line connections at the wellhead and battery. At the time of this inspection, no further action had been taken by Future Production Inc. regarding the stipulations of the associated Chiefs Order. There was no current liability insurance.
