There are four in-ground concrete, brine tanks located adjacent to the production tanks. The wellhead includes a mechanical plunger lift located remotely, across the road. The tank battery is shared with the Lassnick # 4 Unit. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery but there is no ID at the wellhead. I did not observe any leaks, spills, or other signs of contamination. No violations noted. |

The well and battery are located on the grounds of a former commercial campground facility which is no longer functional. Access to the well is afforded by the former unpaved campground access road which enters from Seeley Rd. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system, a tank battery comprised of two 210-barrel poly production tanks, a series of four in-ground concrete brine tanks, and separators. The battery and well are in separate chain-link enclosures. The battery is shared with the Lassnick #4 (2-0838) which is located across Seeley Road. The load-outs were bull-plugged and secure. Secondary containment is intact. I walked the areas surrounding both the wellhead and tank battery and did not observe any evidence of impact or problems associated with the production activity. |

Numerous calls to the owner about the issue with signage at this well as well as many other wells have gone unheeded. |
