0089604093 2017-09-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines were not secured with locks and the tank spacing within the battery was not compliant . In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. During a subsequent inspection, I had observed that the production tank had become corroded with several perforating holes rendering the tank not capable of preventing a release. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
0484686782 2016-02-16 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger-lift system in a chain-link enclosure which is locked. The enclosure is located in an otherwise undeveloped parcel in an area of mostly light industrial development. The tank battery is comprised of one 100-barrel and one 50-barrel tank, and mechanical separator in a chain-link enclosure. Fittings and connections at the battery were secure. Tank lids were closed and secure. Enclosure gates were not locked. Load line valves were not secure with locks. The tank battery is located less than the required minimum distance of 50 feet from the well head. The owner must remedy the following by 04/04/2016: Lock the enclosure gates and load line valves. Correct the tank spacing at the next well work-over activity. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
0596682307 2017-04-19 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves and violation of the tank spacing requirements. At the time of this inspection, I observed the following: The load-line valves were not secured with locks and the production tank remained less than 10 feet from the mechanical separator unit. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I also noted that the production tank had a perforation resulting gin complete loss of integrity. I issued a separate compliance notice for the tank. I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.

At the time of this inspection, I observed the following: There was an approximate 1 cm diameter hole in the production tank from which oil has been leaking. I contacted John Magnussen with Rockefeller Oil and advised him about the tank requiring repair or replacement. Im advising the well owner to remedy the leaking tank by 05/26/17. 1378836132 2013-09-10 UrbanProduction Wells No Violations The producing unit consists of a wellhead with mechanical plunger lift system and aboveground storage tank battery. The battery, separator, and wellhead are in separate chain-link enclosures. The battery is comprised of 1 100-barrel fiber-reinforced plastic (FRP) production tank, 1 50-barrel steel brine tank, and separator unit. There is adequate secondary containment. There is no safety equipment as required by 1501:9-9-05E. Locks and signage are present. The ID placard has the wrong API #. The locked gated entry to the access road also requires a sign.
1404824453 2014-07-07 UrbanProduction Wells No Violations I inspected this location pursuant to a notice of a possible fire and subsequent confirmation with the Lake County Emergency Management Agency at 8:45 PM. Upon my arrival, crews from the City of Mentor Fire Department (MFD) and from Great Plains Exploration were on site. According to MFD site commander, it is believed that lightning struck a portion of the tank battery resulting in a fire associated with the 100-barrel brine tank, which is comprised of fiber-reinforced plastic (FRP). The top couple of feet of the tank were damaged, burned and/or melted. The resultant fire was extinguished with foam by crews from MFD. The fire did not appear to affect any other portion of the system, and damage was limited to the one tank. I observed some ponded water/foam fluids (presumably from the fire response activity) within the diked area. It appeared that several inches of oil had been floating on the brine liquid in the tank, and subsequently leaked down the tank sides after the fire. It did not appear that any liquids breached the diked area. During my time on location, crews from Great Plains arrived with a water truck and pumped out the remaining liquid from the damaged tank. John Magnuson of Great Plains reported that they will dismantle and remove the damaged tank tomorrow (7/8) and replace it before returning the unit to service. I requested a copy of MFDs report of the incident. I will re-inspect tomorrow and assess what, if any, additional site mitigation or clean-up is warranted.
1554229948 2019-04-02 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice #1215468600, regarding the removal of the 100 BBL tank that had a previous leak, updating identification and spacing issues between the well and production equipment. At the time of this inspection, the locks had not been installed at the load-line valves and the tank spacing requirements have not been met. The 100 BBL tank has not been removed due to the previous leak from an approximate 1 cm diameter hole in the production tank. Identification still needs to be updated to reflect the current owner information.

I also had previously noted that the spacing from the tank to the separator, and from the battery to the wellhead did not meet the minimum spacing requirements. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
1951351731 2014-07-09 UrbanProduction Wells No Violations I inspected this unit as a follow-up to a lightning strike and resultant fire that occurred on 7/7/14. The fire damaged the fiber-reinforced plastic (FRP) production tank and resulted in a small amount of oil to spill down the tank wall sides, which was contained within the diked area. At the time of my inspection, crews from Great Plains Exploration are bringing in a new, steel replacement tank. I discussed with John Magnussen from Great Plains the requirement to maintain 10 feet of clearance between the production tank and the separator. Also, I advised him that the battery needs to have safety equipment (spark arrestor, low-pressure relief valve, and carbon filter). Crews are working to clean up the battery and make repairs.
