At the time of the previous inspections, I had noted that the well was idle and had not been produced or had annual production reported since at least 2009. At the time of this inspection, I observed the following: a wellhead with 8.625-inch surface casing with 4.5 to 2 reducer swage and flow-line T-style connection. The existing flow-line connection valve was closed. The well was not being produced. The area around the wellhead was significantly overgrown with small trees and other dense vegetation. Since the time of the previous inspection, there have been no changes and no attempts to plug and abandon the well.

The well is in a wooded area located between residential and commercial properties along Fairport-Nursery Road and the Grand River. At the time of this inspection, the visible portion of the well is comprised of a 4.5-inch production casing with a 4.5 to 2 reducer swage and closure gate valve which is closed. The area surrounding is comprised of tall grassy vegetation and woods. There are no aboveground production tanks or other appurtenances apparent. There has been no production reported and no annual production reports filed since 2008. The well owner/operator shall submit a written plugging schedule to Inspector Stephen Tompkins at 3575 Forest Lake Drive., Suite 150, Uniontown, OH, 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
