The wellhead is located on a shared road within the Losely Nursery facility, which enters at Shepard Road. The wellhead is comprised of a 4.5-inch riser with a closure valve which is shut-in. There are the remains of a pump jack platform. The pump jack has been removed. The well is not connected to any production device or flow line. There has no been production reported since 2005. The well owner must plug and abandon the well in accordance with ODNR rules and requirements within 180 days or be subject to additional enforcement action.
0540779060 2015-10-29 UrbanProduction Wells No Violations I inspected this well as a follow-up to a Compliance Notice issued on 11/14/2014. The well has not had reported production since 2005 and therefore the owner was directed to plug this well by 05/15/2015. As of the date of this inspection, the well remains in the state in which was observed at the time of the issuance of the Compliance Notice. I will refer this matter for additional enforcement action in the form of Chiefs Order.
0560205768 2018-05-09 UrbanProduction Wells No Violations I inspected this well as a follow-up to a Compliance Notice issued on 11/14/2014. The well has not had reported production since 2005 and therefore the owner was directed to plug this well by 05/15/2015. As of the date of this inspection, the well remains in the state in which was observed at the time of the issuance of the Compliance Notice. Since the issuance of the Compliance Notice, the ownership of the well has transferred to STMX Gas, Inc. I contacted Brent Van Sciok of STMX Gas Inc. regarding the plans for returning the well to active production. He indicated that STMX plans to return this well to production, along with the remaining idle wells that STMX acquired from Autumn Ridge, within 6 months.
0946276739 2015-03-10 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and related Compliance Notice issued on 11/14/2014. The well is idle and has not had production reported since 2005. No action has been taken at the time of this inspection to procure a permit to plug or otherwise return the well to production. Ive contatced Autumn Ridge as of this date for an update and am awaiting a response.

At the time of this inspection, there remained no production equipment. The well was unable to be produced. Autumn Ridge had made no progress towards returning the well to production. Subsequent conversations with the owner have not produced a definitive plan for compliance.

Repeated calls to the owner have not produced any demonstrable action or results.
