At the time of this inspection, I observed the following: the load line valves and gate were not locked and the ID signage remained out of compliance I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.

The well is located behind a light industrial facility on Heisley Rd. in a level grassy area just off the parking lot. There is a stuffing box fitting at the wellhead and sucker rods, but no other production equipment. The well is currently producing from the tubing. The battery is comprised of one 100-barrel steel production tank and one 100-barrel FRP brine tank, and separator. Connections at the wellhead and battery are secure and free from signs of leakage or problems. I did not observe any evidence of impact relating to on-going production activity.
1378221346 2013-09-03 UrbanProduction Wells No Violations The producing unit consists of a wellhead and aboveground storage tank (AST) battery. The wellhead formerly was fitted with a pumpjack unit. There are currently rods and stuffing box fitting only now. The battery is comprised of one fiber-reinforced plastic (FRP) and one steel production tank, and a separator unit. I spoke with Matt from Great Plains regarding the well status. He indicated that a new pumpjack unit was being readied for deployment at this well site.

At the time of this inspection I observed the following elements: A wellhead, a mechanical separator, and a tank battery system in a open area behind an industrial facility. Access to the wellhead is via the shared, asphalt-paved driveway. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. The load-line valves are not secured with locks as required. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks. At the time of this inspection, I observed the following: The wellhead was comprised of a valve with flow-line and regulator attachment but the pump-jack system had been removed previously. The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
