0645069699 2012-03-19 UrbanProduction Wells No Violations My inspection today found that the stuffing box had leaked 2 to 3 gallons of crude oil around the well head. I applied sorbent pads to the affected area and stopped the oil from flowing away from the well head. I contacted the well owner and informed them of the problem. The well tender was on site within one half hour and shut the well down, cleaned up free oil/spent pads and mixed Peatsorb into the soil around the well head. The total area affected from the leak was approximately 80 square feet. I informed the well owner, during this inspection, that locks are needed on the load line valves at the tank battery. The tank battery is not fenced and there is no HB-278 equipment installed on the tank battery. Preliminary Restoration was passed by a senior inspector in 2009.

1336995622 2012-05-10 Production Wells Violations Noted On 5/10/2012, I performed a production well inspection status check with DNR Inspectors Tom Hill and Mike Dinsmore. My inspection found the pumping unit had a small drainage ditch within 5 feet of the well head. DNR Inspector Tom Hill called Great Plains Exploration and instructed the well tender to put a dike around the well head. The tank battery contained one 210 barrel oil storage tank, one 100 barrel brine tank, and a separator and I noticed no visible spills or leaks. The lease road was in good condition. I will perform a follow-up inspection by 5/18/2012.

The operator has a dike built around the wellhead, that is 2/3 completed. Operator was contacted to close of dike to prevent fluid release from the well, which is equipped with a pumping unit. The peat-sorb that has been mixed into the soil during spill clean up has stabilized the remaining free crude oil on location. Operator was contacted to remove remaining saturated peat-sorb. 1482275234 2016-12-20 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had observed that there was oil leaking from a back-pressure valve and from gas leaking from a fitting at the wellhead. I had notified the company and they had indicated that they would address the leaks. At the time of the this inspection, the previously-noted leaks had been repaired and there was no visible oily impact to the ground surface. Fittings and connections at the tank battery and wellhead were secure. Secondary containment was intact and dry. The access road was mostly-snow covered but passable.

1917993732 2016-02-29 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following elements: one wellhead with pump jack system, one mechanical separator, and a tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, and mechanical separator, with associated fittings and tubular connections. Secondary containment was intact. Access to the well is afforded by a short dedicated, unpaved access road which enters from Chardon Road. At the time of this inspection, the road was in good condition. I observed the following deficiency: oil was leaking from the back-pressure valve at the wellhead forming a small (less than 8 oz.) puddle at the base of the fitting. In addition, I observed gas leaking from a fitting at the wellhead near the drip tank and gas connection for the pump jack motor. I contacted Keith Krejci with Great Plains Exploration and advised that repairs must be made immediately. The owner must remedy the following by 02/30/2016: Fix the leaking oil from the back-pressure valve and the leaking gas connection. The spacing is in accordance with the rule, and the valving and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a pump jack which is producing. The tank battery dike is adequate. Locks and ID are good. No violations noted.
