I applied sorbent pads to the affected area and stopped the oil from flowing away from the well head. I contacted the well owner and informed them of the problem. The well tender was on site within one half hour and shut the well down, cleaned up free oil/spent pads and mixed Peatsorb into the soil around the well head. The total area affected from the leak was approximately 80 square feet. I informed the well owner, during this inspection, that locks are needed on the load line valves at the tank battery. The tank battery is not fenced and there is no HB-278 equipment installed on the tank battery. Preliminary Restoration was passed by a senior inspector in 2009. |

The tank battery contained one 210 barrel oil storage tank, one 100 barrel brine tank, and a separator and I noticed no visible spills or leaks. The lease road was in good condition. At the time of this inspection, I observed the following elements: one wellhead with pump jack system, one mechanical separator, and a tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, and mechanical separator, with associated fittings and tubular connections. Secondary containment was intact. Access to the well is afforded by a short dedicated, unpaved access road which enters from Chardon Road. At the time of this inspection, the road was in good condition. I observed the following deficiency: oil was leaking from the back-pressure valve at the wellhead forming a small (less than 8 oz.) puddle at the base of the fitting. In addition, I observed gas leaking from a fitting at the wellhead near the drip tank and gas connection for the pump jack motor. I contacted Keith Krejci with Great Plains Exploration and advised that repairs must be made immediately. The owner must remedy the following by 02/30/2016: Fix the leaking oil from the back-pressure valve and the leaking gas connection.
