0020773107 2017-04-14 UrbanProduction Wells No Violations I inspected this producing well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a asphalt-paved road that enters from Norton Parkway and travels 1,800 feet to the well and battery, was in good condition. The access road has been converted to a multi-purpose trail by the landowner and has rendered the well inaccessible, which is addressed under separate report. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety deficiencies; the load line valves remain unsecured with locks, the ID signage had not been updated to reflect the recent change in ownership. In addition, I had noted that the tanks were lacking the required process safety equipment (spark arrestor, low-pressure relief valve, carbon filter). The tank battery enclosure was missing a section of boarding and was not secure. I attempted to contact both company representatives John Magnussen and Richard Osborne and, as of the filing of this report, no representative of the company has responded.

0766610670 2019-03-28 UrbanProduction Wells Violations Noted I was on location regarding complaint # 6117, involving safety issues at the well site location due to the tank battery fencing not secured. There is currently an open compliance notice # 902968763, regarding the installation of locks on the tank valves. At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The gate valve and 2 inch flow line valve were closed. The well was enclosed with board on board fence that had a locked man door. The tank battery contained two 100 BBL tanks. The urban safety equipment (low-pressure relief valve, spark arrestor, carbon filter), has not been installed on the production tanks and needs to be installed. The locks required to be installed per the previous compliance notice have not been installed on the tank valves. The board on board enclosure was present around the tanks, but sections of the fencing had fallen down and needed to be repaired. The mechanical separator had closed valves and was also enclosed with a board on board enclosure. The enclosure had a locked man door. The identification located on the separator door did not contain current owner information. Lease access has been removed due to the walking trail that has been installed near the well location. I contacted Rockefeller Oil Co. and spoke with Jenny Corrao regarding the locks for tank valves needing installed and installing the urban safety equipment on the production tanks. I also discussed the need for repairing the fencing around the tank battery and installing new identification reflecting current owner information.
0843382797 2018-06-14 UrbanProduction Wells No Violations I inspected this well location pursuant to a report of a fallen tree that impacted a portion of the tank battery. Upon my inspection, I noted the following: a small tree limb had fallen into the mechanical separator, but had not impacted any of the production elements. I had previously contacted John Magnussen about the tree limb, and he indicated that he would tend to it. The well was shut-in and not being actively produced.

At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift and tank battery consisting of three 100-barrel steel, storage tanks, with mechanical separator. The battery and wellhead were in in separate board-on-board enclosures. Fittings and connections are free from leakage. The dedicated access road is in good condition. The enclosure gates were locked and secure. The load-line valves were not locked and must be secured with locks. The owner must remedy the following by 04/01/2016: secure the load-line valves and enclosure gates with locks. I did not observe any signs of leakage or problems at the wellhead. The tank battery is comprised of 3 100-barrel steel tanks in a board-on-board enclosure which is locked. A mechanical separator is located in a separate board-on-board enclosure. Secondary containment is present and intact. ID signage is present. Load-outs are bull-plugged. Unit spacing meets requirements. I did not observe signs of leakage, spills, or problems.

At the time of this inspection, I observed the following: the noted deficiencies remained. In addition, the unit ID signage had not been updated to reflect the change in ownership that had occurred in 2016. The well was not in active production and there was not an active chart record. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
1469737205 2016-07-28 UrbanProduction Wells No Violations I inspected this well pursuant to a request from Mr. Dave Swiger from the City of Mentor Engineering department. According to Mr. Swiger, there had been reports that some elements of the fencing at the battery were missing or damaged. Upon my inspection, I observed the following: a portion of the board-on-board fencing was missing and had been replaced with orange snow fencing. Valves at the wellhead were closed and the well was shut-in. The wellhead enclosure was secure and locked as well as the separator enclosure. I advised Mr. Swiger that I would report the condition to the trustee handling the on-going bankruptcy litigation involving Great Plains Exploration.

At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
