0838324772 2019-02-07 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 893080369, regarding installation of urban safety equipment, i.e. low pressure relief valve, spark arrestor and charcoal filter, and install locks on the doors of the fenced enclosures at the well and tank battery, and load lines. At the time of this inspection, the locks had not been installed at the well or tank battery doors and the required urban safety equipment had not been installed on the production tank. The well was equipped with a plunger-lift system and not in production. The fence surrounding the well was intact but still needed to be locked. The tank battery was comprised of two 100 BBL tanks and a mechanical separator. The load lines still had not been fitted with locks. The diked tank battery was surrounded by a fenced enclosure but still needed locked. Access to the well and tank battery was secure.
0893080369 2016-02-29 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following elements: one wellhead with mechanical plunger-lift system in a board-on-board enclosure, one mechanical separator in a board-on-board enclosure, a tank battery comprised of two 100-barrel steel tanks with associated fittings and tubular connections in separate board-on-board enclosures. I observed the system fittings and connections to be secure and free from leakage. The tank lids were closed and secure. Secondary containment was intact. I walked the areas surrounding the wellhead and battery and did not observe any evidence of impact or erosional deficits. Access to the well is afforded by a short dedicated, unpaved access road which enters from Baldwin Road. At the time of this inspection, the road was in good condition. I observed the following safety deficiency: the wellhead enclosure and the mechanical separator gates were not locked and secure, the load-lines valve was not locked as required. Spark arrestor, low-pressure relief valve, and carbon filter were not present as required. The owner must remedy the following by 03/31/2016: secure the enclosure gates and load-lines with locks. Fit the system with spark arrestor, low-pressure relief valve, and carbon filter as required. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

At the time of this inspection, these deficiencies remain. The company has pledged on a repeated basis to remedy the problems but have not acted as of this date.
