0118645190 | 2011-07-28 | UrbanProduction Wells | Violations Noted | Dike and ID in compliance. Vent hatches installed and closed. Feedlines are bullplugged. Wellhead gate is not locked. Lightning arrestor and charcoal filter are not installed. |
0665163475 | 2017-04-06 | UrbanProduction Wells | No Violations | I inspected this well location as a follow-up to a previously-issued Compliance Notice for the following deficiencies: no locks on load-line valves and no gauge on the surface casing annulus as required by permit condition. At the time of this inspection, I found the following: Fittings and connections at the wellhead were secure and free from leakage. The tank battery was comprised of a 100-barrel steel production tank, one 100-barrel steel brine tank, with separator unit located on an otherwise undeveloped parcel in a commercial development. The following conditions remained extant: Load-lines remained unsecure. The surface casing was plumbed to surface but not gauged as required. John Magnusssen with Rockefeller Oil has responded to multiple contacts by saying they are fixing the deficiencies as they are able with their resources at hand. |

1069255594 | 2016-02-09 | UrbanProduction Wells | Violations Noted | I inspected this well as part of routine inspection activity. The well and associated battery are located in an otherwise open, grassy area within a commercial strip of restaurants. The well and battery are accessed via a short, unpaved road that connects to the existing asphalt-paved driveway of the Waffle House Restaurant. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure. Fittings and connections at the wellhead were secure. The 8.625-inch casing is valved at surface but not gauged as required by permit condition. The battery is comprised of two 100-barrel steel production tanks and separator in a separate board-on-board enclosure. Enclosure gates were locked and secure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks. Place and maintain a gauge at the surface casing nipple as required by permit condition. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection. |

1415284834 | 2014-11-06 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. The wellhead plunger lift and battery are located on a short, dedicated access road from CR 400, beside the Waffle House. Fittings and connections at the wellhead are secure and free from signs of leakage or problems. The battery is comprised of 2 100-barrel steel tank, and a separator. Fittings and connections at the battery are free from signs of leakage or problems. Secondary containment is present. ID signage is present. Load-outs are bull-plugged. |

1512486566 | 2017-12-05 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked, and the 8.625-inch casing was not gauged nor fitted with a pressure relief device as required under the special permit conditions which were part of the drilling permit. At the time of this inspection, I observed the following: the ere no locks at the load-line valves, and the surface casing nipple was not gauged or fitted with a pressure relief device. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and lack of funds, have prevented them from being in compliance. |
1518550437 | 2018-02-13 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves were not locked, and the 8.625-inch casing was not gauged nor fitted with a pressure relief device as required under the special permit conditions which were part of the drilling permit. At the time of this inspection, I observed the following: the were no locks at the load-line valves, and the surface casing nipple was not gauged or fitted with a pressure relief device. The ID signage had been updated to reflect the new ownership. I had previously contacted the company on several occasions since the issuance of the compliance notice regarding the deficiencies. The company had made pledges to remedy the deficiencies. |
1548443584 | 2019-01-25 | UrbanProduction Wells | No Violations | I conducted an compliance notice follow-up inspection for compliance notice 1069255594, the load-line valves not being locked, and the 8.625-inch casing was not gauged nor fitted with a pressure relief device as required under the special permit conditions. At the time of this inspection, no locks were installed on the load-line valves, and the surface casing nipple was not gauged or fitted with a pressure relief device.
Rockefeller Oil Company, LLC still needs to: 1) Install locks on the load-line valves on the tanks 2) The surface casing on the well needs fitted with a pressure relief device and gauged |
1668130692 | 2013-12-02 | UrbanProduction Wells | Violations Noted | Locks and safety gear in accordance with 1501:9-9-05E has been added. No problems noted. |
1801897476 | 2015-02-13 | UrbanProduction Wells | No Violations | I inspected this producing unit as part of routine inspection activity. The well is located in a commercial and retail development at the intersection of Auburn Rd. and SR 44. The well is comprised of a wellhead with mechanical plunger lift located 200 feet from the tank battery. The battery, mechanical separator, and battery are in separate board-on-board enclosures. The enclosure for the wellhead is unlocked and I found the gate to be left open with the enclosure partially broken down. Load outs at the battery are bull-plugged. Connections and fittings are secure and free from signs of leakage or problems. At the time of this inspection, there are 10 to 15 inches of snowfall, which hinders a thorough inspection of surficial features (e.g. access road, secondary containment interior. I notified Tim Reilly (John D. Oil & Gas) about the need for a lock on the gate. I will follow-up in 30 days. |
