0088374735 2017-12-11 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the 100-barrel brine tank was severely rusted and lost integrity, there was no spark arrestor, low-pressure relief valve, or carbon filter, the load-line valves were not locked and secure, and the unit signage had not been updated since the change of owner that had occurred in 2016. At the time of this inspection, I observed the following: the noted deficiencies remained. In addition, there was not fencing or enclosure at the wellhead or battery. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.

At the time of the previous inspection, I had noted the following: the load-line valves were not secure with locks, the production tank was perforated with numerous holes, and the tanks were missing carbon filter and spark arrestor. At the time of this inspection, the noted deficiencies remain. In addition, the signage has not been updated to reflect the change of ownership that occurred in 12/2016. I contacted John Magnussen with David Oil who indicated that the issues are being handled as they are able. I inspected the wellhead and surrounding area and found no signs of problems otherwise.

1456507760 2016-02-26 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system in a board-on-board enclosure. A tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank, a mechanical separator in separate board-on-board enclosure. The production tank is severely corroded and mechanical integrity has been compromised. The enclosure gates were unlocked and not secure. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. Fittings and connections at the wellhead were otherwise secure and free from signs of leakage or problems. The owner must remedy the following by 04/03/2016: Remove the production tank from service and replace, secure the load-line valves and enclosure gates with locks. Fit the vent stack with spark arrestor, low-pressure relief valve, and carbon filter as required. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
1819960999 2017-04-06 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued Compliance Notice. At the time of this inspection, I found the following: Fittings and connections at the wellhead were secure and free from leakage. The tank battery was comprised of a 210-barrel steel production tank, and 100-barrel steel brine tank with separator unit located in the back portion of a larger rural parcel. The following conditions remained extant: The brine tank that had been noted to be rusting with several perforations remained within the battery but had been disconnected. Load-lines remained unsecure. Safety equipment which had been noted to be lacking previously (e.g. spark arrestor, low-pressure relief valve, and carbon filter) remained so. The surface casing was not plumbed to surface and gauged as required. There was no ID signage. John Magnussen indicated that the issues are being addressed as they are able.
