0177020710 2018-03-15 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on a larger, residential parcel. At the time of this inspection, I found the following: a wellhead pump jack unit and associated battery. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The well was in active production. The chart recorder was not active at the time of this inspection. Production reports were current. During a prior inspection, I noted that the insurance certificate had expired as of 8/2017. Since that time, the insurance was restored to current.

0579312150 2014-06-18 UrbanProduction Wells No Violations I inspected the metering run associated with this unit as a follow-up to an earlier complaint regarding an unlabled drum being stored there. Pursuant to my contact with Petro Evaluation the drum has been removed.
1400865210 2014-05-23 UrbanProduction Wells No Violations I inspected the producing unit pursuant to a complaint registered by the Lake County Metro Parks regarding an labeled drum located on park property, and associated with production appurtenances. Upon inspection of the location of the complaint (Lake County Metro Parks Education Building located at 9045 Balwin Road), I observed a natural gas metering run near the road. Next to the metering run I observed an unlabeled blue 55-gallon drum. At the time of my inspection the drum is full with what appears to be oil collected from the drip tank at the metering run. I did not see any evidence of leakage from the drum but it is in poor condition. I contacted Steve Franks with Petro Evaluation Services and advised him that the drum should be removed for proper disposal. Conditions at the wellsite and tank battery are otherwise in good condition. The unit is producing with no signs of leaks, spills, or problems.
1403036277 2014-06-17 UrbanProduction Wells No Violations I inspected the unit pursuant to an earlier complaint regarding an unlabeled drum being used and/or stored at the metering run on Baldwin Rd. Subsequent contact with Steve Franks of Petrol Eval. Had indicated that the drum had been removed as requested, however I received a complaint today that the drum is still at that location. Upon my arrival at the metering run, I observed the drum and contacted Steve Franks. I requested that the drum be removed for disposal within 24 hours. He indciated that he would take care of it. I will follow-up tomorrow.

1497633409 2017-06-16 Production Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery located at the back portion of a larger residential parcel. Fittings at the wellhead and tank battery were secure and free from signs of leakage or problems. Tank lids were closed. Load lines were closed, plugged and secure. The was intact secondary containment which was dry and free from any significant signs of spills, leakage, or other problems. I walked the areas surrounding the well and battery and observed grassy and wooded areas with no signs of impact or degradation to the natural greenscape. The gravel-paved access road was in good condition with no erosional deficits. Production data was current and there were no indications of problems or deficiencies regarding bonding and insurance.

I contacted ODNR Surety and Bonding in Columbus as well as Steve Franks with Petro Evaluation Service, Inc. regarding the insurance. On 11/29/17, Surety Bonding indicated that they were in receipt of a new, updated certificate.
