0101339362 2012-03-30 UrbanProduction Wells No Violations I was on site with DNR Supervisor Gene Chini. I tested the water at the discharge of the culvert north/east of the wellhead for Chlorides with high range Quantabs. Readings were 0 PPM. This is the location of the beige colored suspended material described in previous inspections. The water level in the ditch has decreased since my last inspection. An inspection of the ditch, up stream of the culvert, revealed this beige colored suspended material in the bottom of the stream. The west bank of this ditch is concrete fill material. This sediment appears to be fines from the fill material. I collected soil samples, using a soil probe, west and northwest of the wellhead. The test holes west of the wellhead indicated the soil was primarily clay. Test holes to the northwest of the wellhead found the soil at the base of the hole was primarily clay with a silty clay in the top 12 of the hole. Salt crystals were noted on the soil in front of the wellhead.
0243094272 2017-05-01 Final Restoration No Violations I inspected this former well location as an evaluation for final restoration. At the time of this inspection, the well was plugged and abandoned with plugging report filed. The production equipment had been removed and the former locations of the wellhead and battery were graded level. The landowner has agreed to waive the requirements to conduct any further restoration. No re-seeding or removal of the lease road is required and a waiver has been submitted. Final Restoration is complete.

0893397594 2012-03-14 UrbanProduction Wells No Violations My inspection of this well site found that the well has been converted from a plunger lift system to a pumping unit. It appears that the stuffing box had leaked and there is a brine scald around the wellhead. The stream to the north east of the well head has cream colored suspended solids in it. There are pools of what appears to be blue green algae growing in the water and there is Iron bacteria is evident. The water and the solids separate when sample is taken. I tested the chloride levels in a pool of the blue green material - > 1600 PPM. I also tested the chloride levels at the discharge end of the culvert - < 233 PPM. The water is clear entering the culvert but has the solids at the discharge end of the culvert. Water samples were collected at the entrance and discharge end of the culvert and sent to our lab.
0915352705 2017-01-25 Plug / Plug Back No Violations I inspected this well location as a follow-up to plugging and abandonment which was completed on the previous day (01/24/2017). At the time of this inspection, the well casing riser remained intact pending eventual cutting to 30 inches below grade and placement of ID and lid as required. There was no gas being emitted from the well. The area around the well was graded level. The tank battery remained in place pending removal.

1332358735 2012-03-21 UrbanProduction Wells Violations Noted I was on site today with DNR Inspectors Barry Grimm and Mark Moreland to drill one (1) control test hole and four (4) test soil borings on the former pit area, to determine the effectiveness of the solidification. I tested the standing water on the pit area for chlorides (Cl) which were 187 PPM. The GPS location of the water sample was at N41.62651, W081.44322. In addition to the Cl in the standing water, there is brine scalding on the soil on top of the former drill pit area and the vegetative coverage on this area is sparse.

Test hole (TH) #1 was the control test hole and was located 52 south of the south west corner of the tank battery fence (GPS -

TH #2 through TH #4 were drilled to four feet in depth.

TH #2 was drilled 6210 southwest of the wellhead (GPS - N41.62656, W081.44323). The collected sample (#3) showed wet and poorly solidified drill cuttings, wet clay and wet solidification material. This material was tested for Cl-1063 PPM.

TH #3 was drilled 48 3 south west of the wellhead (GPS - N41.62659, W 81.44322). Sample #4 consisted of damp silt/ clay with oxidized (red) sandstone. TH #4 was drilled 41 6 southwest of the wellhead (GPS - N41.62665, W081.44324). Soil sample #5, which was somewhat damp, consisted of silt/ clay with oxidized (red) sandstone and drill mud.

An attempt was made to drill TH #5 (GPS -- N041.62667, W081.44310. A beige colored suspended material was present in the stream, which was tested with Phenolphthalein (1%). Both the water and suspended material reacted with Phenolphthalein, turning pink. Brine s

1332415985 2012-03-21 Production Wells No Violations I arrived on site with DNR Inspectors Grimm and Hill to collect samples. The location is in a wetlands. We completed a 360 * review of the site and found that there are cattails completely surrounding the location. There are two diversion swales that drain water away from the drilling site. During our soil sampling of the drilling pit area, a Quantab was used to test Chlorides on the soil sample. The Chloride reading was over 1000 ppm. After sampling was complete we were able to review the permit information. We found that the permit is a Lake Plains Region permit. As part of the permit conditons the pits should have been solidified and removed after drilling was completed. We met with the Great Plains field supervisor and advised him that there is brine contamination of the surface area in the vicinity of the former drilling pit. We also advised him that there is Chloride contamination entering a drain tile and leaving the site. The Chloride range leaving the site was over 1800 ppm. We advised the field supervisor to excavate the drilling pits and remove any contaminated soil and dispose of it properly. The drain tile also needs to be cleaned out and flushed.
1332432302 2012-03-22 UrbanProduction Wells No Violations I was on-site today to test the surface water, around the location, for chlorides. The testing was conducted using Quan tabs from low range bottle #1137 (expiration date 2013/02) and high range bottle #1099 (expiration date 2013/03). There are three drainage ditches around the location. Ditch #1 is located west of the wellhead and flows south to north and then turns north east and flows under the location through a culvert, which is located approximately 10 yards north of the wellhead and discharges into Ditch #3. This is the culvert that has the sediment issue at the east (discharge) end of it. Ditch #2 is located to the south of the tank battery and flows to the northeast into Ditch #3. Ditch #3 is located on the east side of the well head and tank battery and flows south to north. Drainage direction was confirmed with Bright Byes Tablets. A water sample was collected and tested from Ditch#1 discharge at GPS- N41.62606, W081.44359. Chloride levels were 172 PPM. Another water sample was collected at the culvert entrance to Ditch #1 at GPS- N41.62667, W081.44341. Chloride levels were 158 PPM. Drainage entering the ditches was clear. A water sample was also collected at the intersection of Ditch #1 and #3 at Chlorides at the discharge of the culvert, intersection ditch #1 and ditch #3 at GPS- N41.62667 W081.44310. Chloride levels were 765 PPM. On 03/21/2012 a water sample was tested at this same location and had chloride levels of 236 PPM. A water sample collected at the entrance to Ditch #2 (GPS- N41.62621, W081.44313) had chloride levels of 0 PPM. A water sample was also collected at the discharge of the culvert of Ditch #2 (GPS- N41.62625, W081.44306). Chloride levels were 28 PPM. None of the water in the ditches reacted with Phenolphthalein however, the beige colored suspende
1333653886 2012-04-05 UrbanProduction Wells No Violations When I arrived onsite the operator was pulling vegetation blockages out of the drainage ditch that runs north/south on the east side of the location. This material was stockpiled on location and I tested it for chlorides (1063 PPM). The operator also excavated fill material away from the culvert until clean clay was encountered. I tested the clear water, seeping out of the fill, for chlorides (1063 PPM). This water was clear, with no visible drill mud. I had the operator excavate a test hole (TH) west of the wellhead and 15 north of the pits (TH #1) which is at the interface of the fill and virgin soil. Once the operator excavated below the fill, water began to seep in from the north. The soil to the south of the culvert was dry and dark brown in color. I had the operator excavate a test trench 5 north of the pits (TH #2,), to the same depth as TH #1. The soil in TH #2 was tight, dry clay. This trench was allowed to stand static and remained dry. I had the operator excavate TH #3 in the north end of the former pit. There was solidified material and shale cutting in this TH and it eventually filled with water. The operator backfilled all test holes and the area around the culvert with clean fill. The backfilled areas were seeded and fertilized at the time of inspection and silt fencing was installed. I will continue to monitor this site. If continued chloride leaching is detected, the material in the former pit will have to be removed and disposed of properly.

1423148635 2015-02-05 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. My inspection found the following: Access to the well is via the former car dealer facility parking area. At the time of this inspection, there are 14+ inches of snow cover in the area. As such, access to the wellhead and battery are accessible by foot only. Production equipment has been removed from the wellhead. Only a closure swage and valve remain. The battery is idle. Connections are secure. Production records indicate the well has not produced since 2012. I will follow up with Osair with regards to future plans to plug or produce the well.

The pumping unit has been dismantled and removed. The flow line is disconnected. The tank battery has become overgrown with vegetation. The well has not produced sellable quantities since 2011. The well owner must obtain a permit to plug and abandon this well and execute activities to complete plugging and abandonment by 01/22/2016 or be subject to additional enforcement action. At the time of the previous inspection, I had noted that the well had not had reportable production since 2011. At the time of this inspection, no changes to the condition or production status had been made. Existing elements consist of the following: one wellhead with closure swage at the 4.5-inch casing which was closed. Flow-line connections were disconnected. The tank battery was observed to be comprised of two 100-barrel steel tanks and associated separator. The area was overgrown with vegetation. The well is not capable of production.
1472212608 2017-01-24 Plug / Plug Back No Violations I was at this location to witness on-going plugging and abandonment activities which were commecnced on 01/24/2017. Upon inspection at the beginning of the day, the bottom formation plug was found to be in place, stable, and the well was free from gas production from the 4.5-inch casing. The 4.5-inch casing was mechanically ripped at 1,840 feet and the remaining staged plugs were set through the 4.5-inch casing during removal. Activities proceeded without incident with the following exception: during hole circulation ahead of the placement of the Big Lime Formation plug at 1575 feet, hydrogen sulfide gas was encountered off-gassing from the circulation fluids for several minutes. Levels at the top of the circulation pit ranged from 50 to 95 ppm hydrogen sulfide for several minutes and eventually dissipated to zero within 15 to 20 minutes, during which circulation os maintained. Observed levels of hydrogen sulfide around the well, and other areas of work zones did not exceed 2 ppm and also dissipated to zero readings after several minutes. No further detections of hydrogen sulfide or other meter-detectable gases were observed during the remainder of the site activity. The plugging was completed with surface plug being stable at completion.
1485446875 2017-01-23 Plug / Plug Back No Violations I was on location to witness plugging and abandonment activity being conducted by Nicks Well Plugging under contract to Osair, Inc. Details of the plugging operation can be found in the plugging and abandonment report. At the time of this inspection, the production equipment had been removed from the wellhead leaving only a surface casing riser and reducer swage at the 4.5-inch casing. The execution of plugging was to include the placement of serially-staged plugs at the Clinton and Big Lime Formations, and at the surface. Activities completed during my inspection consisted of placement of the bottom (Clinton Formation) plug followed by shut-in of the well over night.
1518679401 2017-01-26 Plug / Plug Back No Violations I was at this well location as a follow-up to plugging and abandonment activity that was completed on 01/24/2017. At the time of this inspection, I met with ODNR Division surveyor Rob Lowe to collect survey data about the well prior to eventual burial. The well was stable with no gas being emitted from the well. Final restoration was still pending at the time of this inspection.
