0734715222 2014-06-15 Orphan No Violations I inspected this location as a follow-up to an earlier inspection and evaluation of current status. At the time of this inspection, I observed a semi-circular area of stressed grass vegetation in the front yard area relative to the inferred location of a well, which had been previously exposed and identified. Near the well is a steel vent stack riser from which some detectable gas was being emitted. The size of the impacted area appears similar to my previous inspections. I did not note any meter-detectable gas readings in the area of the impacted grass area or observe any gas odors.
1449245979 2015-12-03 Orphan No Violations I inspected this well location as an evaluation of current site conditions. The well is located in the front yard area of the referenced house (buried under the existing turf grass). There is a 2-inch steel vent stack riser located near the edge of the property near Dewey. At the time of this inspection, the only surface expression of the well is a 5 diameter dead spot in the grass. There are no meter-detectable gas readings in ambient air at the dead area, at the time of this inspection.

2088706161 2016-06-07 Production Wells No Violations On site this date as part of an investigation to determine whether this well is to be admitted to the Orphan Well Program. Upon speaking with the landowner, he stated that +/- 4 years prior, DEOG exposed this well to determine the cause of an odor complaint. They excavated +/- 6 in depth, exposing what the landowner stated was an iron pipe about 4 in diameter. There was an odor of gas, along with discolored soil, but no fluid present. My inspection revealed no odor of gas to be able to meter, the kill are is 9 X 10, and is located 35 west of the landowners front door in the front yard. Landowner stated that another inspector had been on site some time back.
