1160941230 2018-07-16 Orphan No Violations I was on location to witness plugging and abandonment activity being conducted by Nicks Well Plugging (NWP) under Idle and Orphan well program. At the time of my inspection, crews from NWP were preparing the site which included installing temporary cellar with 60-inch steel corrugated culvert. The sandy subsurface material was slumping around the culvert and the crews were loading imported stone (# 3s and # 4s) as shoring . I met with crew foreman (Jeff) and ODNR Orphan Well Inspector Perry, and reviewed the plugging plan. I noted that there was no meter-detectable gas near the open wellhead. The ingress and egress from the well was loose, sandy material with mats being deployed to stabilize the service rig area.

1467038311 2016-06-24 Orphan No Violations I was on location in reference to an orphan well complaint. Inspector Tompkins initial inspection on 6/7/2016 stated the well casing sizes were documented and photos were taken of the well. The well was reburied for safety purposes. My inspection found the area where the well was located was covered with soil. Photos were taken of the location to start the orphan paperwork.
1492622968 2017-04-19 Orphan No Violations My inspection found the well to be equipped with 10 and 3 casing. The casings have been damaged due to excavation of the property for the construction of a housing development. There is crude oil at the surface between the 10 and 3 casing. The 3 casing is open and fluid is not visible. The soil around the well is saturated with crude oil. The well was not leaking any natural gas at the time of inspection. The well is currently situated on the slope of a drainage ditch and approximately 10 from a retention pond. The drainage ditch and retention pond have been added to the property since the initial inspection. The City of Mentor Fire Department has no known odor complaints associated with the well. There is however an odor complaint on record in 2013 for the producing well permit 2-0414 that is on the property. The well is under review for the orphan program.

1551210969 2016-06-07 Orphan No Violations I inspected this location pursuant to a complaint (#5365) by the owner of the property regarding a poossible well that had been uncovered during preliminary site work for a residential development. Upon inspection, I observed the following: a 10-inch diameter casing open to atmosphere with a 3-inch diameter casing located concentrically wihin. The 3-inch casing extends beyond the terminus of the 10-inch casing by 12 inches. The upper-most 2 feet of the exposed 10-inch casing is severely damaged and deformed rendering inspection of the interior difficult. There appeared to be water in the 10-inch annulus with some floating oil. An occasional gas bubble was noted, but very minimal effluent gas. The top of the casing was approximately 2 feet below the existing grade. The surrounding area is comprised of a generally flat -lying portion of a former greenhouse property. The area was observed to be mostly short, wild field grasses and other vegetation. After inspection, the well was re-covered with loose sandy soils pending further evaluation. There was no identifiable records associated with this suspected well.
