0881708161 2013-11-06 Production Wells Violations Noted The well is set up for domestic gas supply. Trying to contact John Murphy on the disposition of this well. At this moment there is no well identification sign at the wellhead. According to 1501:9-9-05(A)10/1509:23 the wellhead should have attached a sign with at least the following information: current well owner, lease name, well number, permit number, county, and an emergency telephone number. The owner shall install a sign at the wellhead with at least the information listed above. John Murphy, Equity Oil & Gas, contacted me on 11/12/2013. This well is capable of producing domestic supply gas to house above the wellhead according to Murphy. He was going to dispatch his well tender to evaluate pipeline from wellhead to the house meter to check for any leaks in line. According to Nathan Ryan, the homeowner (740-237-6557), the well would build to 500 psi. He would produce gas for three days and the pressure on the gas line to the house would eventually drop to a low pressure where his gas appliances and gas furnace would not work.

1238620481 2009-03-16 Production Wells Violations Noted An inspection was requested by Cheryl Bolender REMAX Agent, for the well located at 908 Twp. Road 266 Kitts Hill, Ohio. An inspection was done on 3-16-2009 at 10:00am. I was accompanied by Craig Ross (Contractor for Equity Oil & Gas Funds). The well does not have correct identification. The well is disconnected from the separator and tank. All valves on wellhead are closed. Well is not leaking gas or oil at the time of inspection. Well is equipped with tubing. I told Craig Ross that they need to place a current identification on well and tank. I told Craig that the well needs to be connected to the production equipment and placed back into production. Place correct identification at wellhead and tank. Well must be capable of production. Connect well to production equipment and connect to tank and production line. Notice of violations were given to Craig Ross in person at the well site.
1287488871 2015-05-01 Production Wells No Violations FOLLOW UP TO NOV # 1145382024. Well equipped with two step down regulators, the valves on the tubing are closed but the well appears capable of production. The 100 bbl tank on site is disconnected and there is a hole in the side. There is a small area (5 x5) of contaminated soils near the bottom of the tank. No meter. No ID.
1396478946 2014-04-02 Production Wells No Violations Upon inspection, this well is set up for domestic gas use, but is currently shut in. There is no identification on the well or the associated (but disconnected) tank, separator, and drip. I contacted Alane King at Equity Oil & Gas Funds and explained that this well needs to be put into production or plugged within the next 60 days. Alane King spoke with their well tender for the area, John Murphy, and contacted me saying they wish to file for a plugging permit for this well.
1410274735 2014-09-09 Production Wells No Violations FOLLOW UP TO NOV # 1145382024. The valves are closed and there is no production equipment at the well. The 100 bbl tank associated with the well is highly corroded and full of holes, the tank has 1-2 feet of fluid in the bottom of it. There is no dike. I spoke with Equity Oil and Gas and the pumper will be coming out to drain the tanks. The company has applied for a permit to plug the well.
