1483111277 2016-12-14 Orphan No Violations Orphan well inspection, accompanied by DOGRM Geologist Jake Glascock: Upon inspection, it is observed that this well is idle and incapable of production. The well consists of 6.5 diameter surface casing, 6.5-4.5 reducing swage, 4.5 well head, and 2.375 production tubing. The tubing is packed off inside the 4.5 well head and is likely set on a packer. The tubing is open to the atmosphere and is extended 5-6 into the air above the top of the well head. No flow lines are attached to the well at any connection. There are no tanks or other production equipment associated with the well. The well is not leaking oil, gas, or brine. The well is located on top of a ridge in an open pasture field. There are no overhead or buried utilities on or near the well location. No trees or other vegetation would need to be cleared to access the well for plugging. The original lease road grade is still intact. The entrance to the well location is through the landowers driveway. R&M Well Service is defunct and this well has been the subject of two separate Chiefs Orders to plug or produce. The well was referred to the field for Orphan Well Program referral in 2006. The Site Evaluation Matrix score for the well is 81. Photos were taken.
