1545231761 2018-12-19 Production Wells No Violations An oil sheen was observed at GPS 38.63143/W082.44574 in Buckeye Creek. The sheen was tracked back up stream to an intermittent stream flowing into Buckeye creek at GPS N38.63069/W082.44105. The sheen and oil release were tracked back to a small wood structure at GPS N38.63459/W082.44105. Inside the structure is a 2 inch line manifold. The manifold is the source of the oil release. I talked to Pillar well tender Jeff and Pillar Field supervisor Mike Carr. The oil release was first noticed by Jeff on Monday 12-17-2018. The release was not reported to ODNR DOGRM One call system by Pillar Energy. The incident was reported by The ODNR DOGRM UIC Inspector on 12-19-2018.
1865453940 2019-02-05 Production Wells No Violations My inspection revealed the surface of the land and the unnamed creek was free of oil. No sign of oil or oil sheen was observed. The soil that was contaminated with oil is held within a plastic lined dike and the dike is covered with plastic. This soil is being bio remediated. The plastic covering the soil is secured outside the dike to allow water to shed off the plastic and not enter into the dike that contains the soil.
1926555572 2018-12-19 Production Wells No Violations I received a call from ODNR emergency response at 10:15 AM alerting me to the fact that oil had been released from a area where 6 wells meet and are tied into a manifold. The leak was in a 2 inch diameter steel line and the oil came to surface where the steel pipe entered into the ground. The oil ran downstream and entered into an unnamed stream. My observation when I arrived was Pillar Energy had the leak uncovered and had absorbent diapers and booms in the creek. I was accompted by ODNR emergency response personal Mr. Nate Lane. We walked the creek and found the devices that Pillar Energy had used to contain the oil were working. They contained the oil in 6 separate location, the last location being before this unnamed stream emptied into buckeye creek. I instructed Mr. Mike Carr with Pillar Energy to start at the point where the oil entered the creek and collect any leaf debris that contained oil. During my inspection Pillar Energy had a trac hoe removing the oil from the surface of the ground. This oil soaked soil was placed in a plastic lined pit and will be covered before 12.20.18 when rain is forecast. Mr. Nate Lane has been in contact with Ohio EPA emergency response personal Mr. Trevor Erwin regarding the measures taken to collect the oil that enter the unnamed stream.

(A) 1501:9-8-2: A reporting person shall notify the division by means of a toll free telephone number designated by the chief and posted on the divisions website or by electronic means designated by the chief and posted on the divisions website within thirty minutes after becoming aware of the occurrence of any of the following unless notification within that time is impracticable under the circumstances: Violation:

1501:9-8-2 (A)(5) Failure to Notify within 30 mins. in a 100-yr flood hazard on FEMA flood insurance rate map: Release of Oil, Condensate, or Materials Saturated with Oil or Condensate; 25 gal. or >in 24-hr period; & release is outside 2nd containment & into environment
