0515978845 2018-07-19 Plug / Plug Back No Violations My inspection revealed that Nethers Bros. has put their rig on the well. There was no one else present during the inspection. I observed that a tubing elevator was connected to 2 inch tubing. During a phone conversation prior to this inspection I was told by Gary Nethers that the tubing is on a packer. The packer had a 1 inch tubing vent connected to it. Gary told me they recovered approximately 500 feet of 1 inch tubing. That would put the packer inside open hole approximately 123 feet below the bottom of the 6.25 inch surface casing. Gary said that they have attempted to pull the packer with two 40 ton casing jacks but could not get the packer to move. I told Gary not to pull any more as it could part the tubing above the packer. I advised Gary to leave the tubing intact as we will need to attempt to pump a cement plug on bottom. During the inspection I observed the 1 inch tubing that was recovered on a trailer. Time of inspection: 12:20 PM. Weather: 79 degrees F, Sunny

1189099776 2012-03-29 Production Wells No Violations An inspection of this well was requested and conducted with Mr. Tom Nethers, in order to verify location prior to plugging and abandonment. A GPS reading was obtained for the well.
1427787523 2016-11-14 Production Wells No Violations This inspection was conducted as a status check. I was accompanied by a prospective new landowner who had some questions and concerns regarding the well. The well was idle during inspection. The well owner will be notified of this inspection.
1532098928 2018-07-20 Plug / Plug Back No Violations During this inspection I witnessed Gary Nethers pull the tubing and recover the packer. Gary planned on running a sand line with a rod on the end down the tubing to check if the tubing was clear. The tubing elevators were still on the tubing when we arrive to the well. When Gary lifted up on the brake handle to lower the tubing elevators to take them off the tubing also dropped. Gary said that after they tried pulling on the packer the other day and letting it sit it the packer must have came loose. Gary then was able to pull the tubing and recovered the packer. Gary will now be able to plug the well with 9 sack grout. Time of inspection: 8:10 AM. Weather: 70 degrees F, Rain
1949437340 2017-08-02 Production Wells No Violations This inspection was conducted as a status check. A plug permit was issued on 7/20/2017. The company intends to plug this well by the end of summer. Time of inspection: 1:20 PM. Weather: 83*F, Partly cloudy
1953814056 2018-08-15 Plug / Plug Back No Violations I witnessed plugging operations on this well today. Gary Nethers used company tools to plug the well. Mini Mix Concrete was used for cementing services. The well was filled solid with 9 sack grout from total depth to surface. The surface casing could not be recovered. Refer to plug report for details. Time of inspection: 1:25 PM.
