0046468006 2002-07-31 Production Wells No Violations At 1:20 P.M. this date I received a call from Jeff Walker Director for Licking County EMA and was advised that there is oil leaking from a tank at the above address, and it is moving toward a water way. At 2:45 P.M. I meet Mr. Walker and Mr. Steve Smith (Deputy Director w/ Licking Co. EMA) on site. Inspection revealed a 100 Bbl. Storage tank has oil running out the top down the side of the tank and onto the ground surface. Oil pads has been put down at the tank. There is approximately three (3) to four (4) gallons of oil on the ground surface. The oil has not made it to a ditch as of this date. There is no identification on the well and/or the two (2) 100 Bbl. Storage tanks. I took GPS of the well and my files revealed this well has an UNKNOWN owner. Mr. Walker advised me that he was advised that Everett Oil might be the owner. I advised Mr. Walker of the violations and I would try to find the owner. I then spoke with Ms. Ball the current land owner and was advised that Edward H. Everett Oil Co. is the owner.

The new ID sign on the saltwater storage tank is incomplete. (no permit, lease name, well number, and county.) WEATHER CONDITIONS: Sunny and 75 degrees.
