1160919980 2012-06-26 Production Wells No Violations The Division received a complaint alleging the disruption of a water well from an idle oil and gas well on property. As a result of alleged disruption, wells located within 1/4 miles were inspected. An inspection revealed that this well was idle. The well consisted of rods and tubing with valve closed on the casing side of well. No house gas appeared to be received. The closest house was equipped with propane. A shared (1) 100 bbl tank with Permit Nos. 2559, and 2476 existed remained on the property. At the tank location, only (1) 1.5 plastic line was observed connected into tank. The line from this well was routed underground and if remaining wells tee into the line this was not located. There was also no observed identificaiton at the well or tank. Records also show no reported production for this well and currently listed as Historic Owner. However, H&D Drilling (aka Harold Donaker) was listed as the original owner. On 7/30/2012, Mr. Harold Donald stated that he owns this well along with the two additional wells on property, Permit Nos. 2465 and 2559., and agrees that all (3) wells on property should be plugged and verbally agreed to resolve non-compliance. A GPS reading was obtained and recorded for the well and tank location. A Compliance Notice will not be issued at this time until the following ownership issue has verified.
1394535619 2015-03-31 Final Restoration No Violations This inspection conducted as a final restoration status check. The well has been plugged. Well location has been reclaimed and production equipment removed. Inspection meets ORC 1509.072 requirements.
1498807283 2014-08-11 Production Wells No Violations This inspection was conducted as a status check. Dalton Lee has moved a service rig on the well in preparation for plugging operations. Mr. Lee was pulling rods and tubing during inspection.
1701314852 2014-08-27 Plug / Plug Back No Violations I witnessed plugging operations today. Dalton Lee plugged this well for H & D Drilling with cement. Refer to plug report for details.
