0162319396 2012-06-26 Production Wells No Violations FOLLOW UP TO NOV # -893555212 The Division received a complaint alleging the disruption of a water well from an idle oil and gas well on property. An inspection revealed an idle well, which consisted of rods and tubing, with no pumping unit or tank(s). Although the pumping unit has been removed from the well, it still remains on the location. The well contained approx 1 plastic off pumping tee side of well and routed directly underground. The valve was also closed on the 1/2 steel line off the casing side of well, which was also routed underground. Black corrugated pipe was observed buried below the ground surface near the well location for drainage (the pipe was not located at the creek). Note: No recent rainfall events at time of inspection. The complainant alleges that water broke-in on casing and the area surrounding the well stays wet for most of the year. No observed meter for commercial sales or house gas received per the complainant. Records also show no history of reported production. There was no observed identificaiton at the well. At 1:45 PM, on 6/25/2012, the well owner, Harold Donaker, was advised of this complaint prior to the inspection and issuance of this Compliance Notice. Mr. Donaker confirmed this well had been idle for approx 12 years and was verbally instructed to plug or produce the well. The well owner shall plug and abandon; produce; or file for Temporary Inactive status within 30 days. If the well were to be placed back into production, identification would be required and statements of production filed for all years of ownership.

1138384761 2006-01-27 Production Wells No Violations Plugging permit expired on 12/21/05. Inspection of well site this date revealed the well is not plugged and is incapable of production. I will call the owner on Monday the 30th of January and advise the owner of the permit status and see what his plains for the well is.
