During the inspection I was accompanied by a prospective buyer of the property who happened to arrive when I did. I contacted Jim Neumeyer during the inspection and advised him that the restoration was due to be completed today per the compliance notice. The well has not been cut down below grade and I observed oilfield equipment around the well location. I spoke with Jim Neumeyer on 5/1/2018 over the phone. I told Mr. Neumeyer that the last time I checked the restoration was not finished. Mr. Neumeyer said that it still hasnt been completed. 1529433081 2018-06-19 Final Restoration Violations Noted My inspection revealed that the final restoration has not been completed. Plugging operations were completed on 11/7/2016 and the final restoration was due to be completed by 5/7/2017. During the inspection I observed the following oil and gas debris and production equipment: metal and plastic lines around the well location, drip, separator and electric line along the lease road. A 2 inch metal line was observed going from the well location to the north where it crosses the Licking River. I observed two 100 barrel tanks that have not been removed. An inspection of the well revealed that the casing has not been cut down below grade and attached with an identification plate. The well owner will also need to fill in the drilling pit next to the well or leave it by having the landowner sign a landowner waiver. The well owner shall finish the final restoration within 15 days to be in compliance. 12 photos were taken of the well location. I contacted Jim Neumeyer of R.H. Penick on 6/20/2018 at 8:54 AM and advised him of my inspection. Time of inspection: 2:00 PM. Weather: 86 degrees F, Sunny
1532448356 2018-07-24 Final Restoration No Violations My inspection revealed that parts of the flow line and electric has not been removed. I advised Jim Neumeyer to spread some grass seed and straw on the tank battery area as well. I did not observe grass seed or straw at the tank battery location. I contacted Jim Neumeyer at 1:15 PM and advised him that the restoration needs finished. Jim said he would have it done by the end of the week. Time of inspection: 12:05 PM. 1534511954 2018-08-17 Final Restoration No Violations My inspection revealed that the restoration has been completed. Jim Neumeyer was finishing the restoration during the inspection. The metal and plastic lines at the well location has been removed as well as the electric line. Jim applied straw and grass seed to the tank battery area. 1930690210 2018-07-18 Final Restoration No Violations My inspection revealed that the two 100 barrel tanks and the dike surrounding the tanks have been removed. The casing has been cut down below grade and covered with dirt. The drip and separator have been removed. I spoke with Jim Neumeyer prior to the inspector as he notified me that this work had been done. He told me he still has some plastic and metal flow line to remove as well as the electric line. I received a signed waiver on 7/13/2018 from the landowner waiving the closing of the drilling pit, removing some of the flow line that cant be recovered, and establishing vegetation on the lease road. Once parts of the flow line and the electric is removed I can pass the final restoration. Time of inspection: 12:40 PM. Weather: 76 degrees F, Partly cloudy
1962707938 2015-03-26 Production Wells No Violations This inspection was conducted as a status check. The well was equipped with a pumping unit and motor. The tank battery consists of (2) 100 barrel tanks inside an earthen dike and (1) 100 barrel tank outside the dike. Identification was not posted. The well is not registered in the correct owners name. The well owner is R.H. Penick Co. and will be advised to place well under there bond. The company will also be advised to post identification.
