0360897203 2016-03-30 Production Wells No Violations This inspection was conducted as a status check. The inspection revealed open 5 casing covered with a 5 gallon bucket. Fluid was observed at approximately 10 feet. A 55 gallon drum and 1 metal line inside plastic PVC pipe stubbed vertically out of the ground was osberved next to the well. A 100 barrel tank was located near the well location. Identification was not observed at the well or tank. The landowner Ms. Virginia Geidenberger alleged the well was last owned by Jack Swick. Ms. Geidenberger also believed that the well had been plugged. Photos were taken of the well and location. Time of inspection: 9:45 AM. Weather: 47*F, Sunny
1465319212 2016-06-03 Production Wells No Violations Well is listed as drilled and Inactive. Well is a Berea well drilled and completed 02/04/1977. Inspected well with DOGRM Licking County Inspector Brandon Mitchell. We met Mrs. Geidenbergers grandson Bradley Wells and his mother Mary Wells who is the executor of the Geidenberger estate. Mary and Brad Wells would like a complaint filed removing the well. We explained the Orphan well program and they would like to make application. A Right of Entry and Plug Application was given to Mary Wells. The well has open 6 out side diameter steel casing, and 7 outside diameter steel casing. Fluid level is visible from the surface and was measured at 10 with a weight and depth meter line. We were able to run the weight to a depth of 610. Division records show a total depth of 725. There was no crude oil visible on the depth counter line. Division records show this well was equipped with 64 of 7,400 of 5 and 720 of 2. There is no visible 2 tubing and this well may have a partial plug in the bottom. Previous Inspection reports indicate that the landowner Don Geidenberger (deceased) thought that the well was plugged. This well is not leaking fluids or gas. There is 1) 100 Barrel steel storage tank that is empty. There is a wooden tower or home made separator next to the storage tank. There is a 55 gallon steel drum next to the well head that has approximately 1 of unknown fluid. There is a white 4 PVC pipe that contains a 1 steel production line and electrical wiring which have been disconnected. I took 20 photos. Brad wells showed us the preferred Right of Entry access and the possible location of a second well.
2062890567 2016-06-03 Orphan No Violations I accompanied Jeff Fry on this inspection. We met with the landowners daughter and grandson at the property to discuss referring this well to the orphan program. Time of inspection: 9:00 AM.
