0005917125 2018-05-09 Production Wells No Violations I conducted a compliance notice follow-up today on the Rector #5 well. I issued a compliance notice to Mr. Douglas Wills on 6/17/2016 to plug the well. My inspection revealed that the well has not been plugged or placed into production. The well was in the exact appearance as the previous inspection. The well owner has not made an attempt to bring the well into compliance. I contacted the well owner Mr. Doug Wills by phone at 3:15 PM. I told Mr. Wills that enforcement will be pursued if the well is not in compliance within the next two months. 0823460998 2018-02-13 Production Wells No Violations I conducted a compliance notice follow-up on this well today. I issued a compliance notice to Mr. Douglas Wills on 6/17/2016 to plug the well. My inspection revealed that the well has not been plugged or placed into production. The well was equipped with a pumping unit and electric motor but did not show signs of recent production. The well was not capable of production as the wiring from the motor was disconnected from the electric panel. The pumping unit was in the same position as it has been for every inspection. I have not received a plugging plan from Mr. Wills. At 12:29 PM I called Mr. Wills and advised him that the well needs to be plugged or put in production. Mr. Wills said he will contact someone to get an estimate on servicing or plugging. I told Mr. Wills that enforcement action may be taken if the well isnt brought into compliance this year. It did not show signs of recent production. The well was not capable of production. Identification was not posted at the well. The tank battery consists of (1) 100 barrel tank and a horzontal steel tank used for brine storage. Identification was not observed at the tank. The RBDMS well status at time of inspection is (ND) Not Drilled. Mr. Doug Wills last reported production on this well in 2010 and indicated (0) barrels of oil and (0) mcf of gas. (3) photos were taken of well. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509.12 (B), and plug the well within 180 days. The plugging contractor said he will contact Mr. Wills about plugging the well. Mr. Wills contacted me on 10/19/2018 regarding the message I left for him the previous day. I advised Mr. Wills that he needs to work on bringing the well into compliance.

My inspection revealed that no attempt has been made to bring the well into compliance. The well has not been plugged or placed into production. I issued a compliance notice to Mr. Douglas Wills on 6/17/2016 to plug the well. I took four photos of the well. During the inspection I observed oil contamination on the ground around the drive pipe. I could see oil between the drive pipe and surface casing. It appears that a hole may have developed somewhere on the surface casing and is allowing oil to enter the anular space between the drive pipe and surface casing. This potentially could be causing contamination within freshwater aquifers downhole. I left a voicemail messagae for Mr. Wills on 3/28/2019 at 9:10 AM. Time of inspection: 12:44 PM.
