I received a landowner waiver signed by the landowner Carl Baker to leave the flow line in place. The well location meets ORC 1509.072 requirements. Time of inspection: 11:55 AM. 0580857613 2017-05-10 Plug / Plug Back No Violations My inspection revealed that the well has been plugged. The plug report will need to be filled out by the owner and notarized.
0801721977 2017-04-28 Plug / Plug Back No Violations My inspection revealed Gene Mathenys service rig on the well for LBJ Drilling. Gene Matheny was contracted to plug this well with clay. Gene Matheny washed in 250 shovels of clay on bottom on 4/27/2017. The 4.5 inch casing was ripped at 2,013 feet. I did not witness any clay washed on bottom.
1143177540 2017-05-02 Plug / Plug Back No Violations My inspection revealed Gene Mathenys service rig on the well for LBJ Drilling. Gene Matheny was contracted to plug this well with clay. Gene Matheny had to rip the 8 inch casing to recover it. Gene recovered 322 feet of 8 inch. I did not witness clay washed in on the top hole plug.
1320335510 2018-07-19 Final Restoration No Violations My inspection revealed that the well has been plugged. The well location has been restored and vegetation established. I observed two wooden electric poles that serviced the well. I spoke with the landowner during the inspection and he told me that the flow line was left in place. I contacted well owner Jeff Moran at 9:55 AM and advised him that that the electric poles need removed and the flow line waived by the landowner to complete the final restoration. I advised Jeff to have this done by 7/27/2018 or a compliance notice will be issued. Time of inspection: 9:35 AM. Weather: 72 degrees F, Sunny
