0127918152 2018-02-09 Final Restoration No Violations I conducted an inspection on this well today and found that the well appears to have been plugged. I observed 10 inch casing at ground level with water inside the casing. A 1.5 inch plastic black pipe was sticking out of the casing. There was an obstruction at approximately 5 feet. I could not find any plugging records on this well in RBDMS. I left a voicemail for Don Dorsey regarding this inspection. Time of inspection: 2:20 PM. Weather: 40*F, Mostly cloudy
0397335943 2018-10-22 Final Restoration No Violations My inspection revealed that the casing has been cut down below grade and covered with soil. A plate with the permit number was placed on the casing. Time of inspection: 1:15 PM.
1538591053 2018-10-03 Plug / Plug Back No Violations The casing has not been cut down and a plug report has not been furnished to the Division. I left some ribbon at the well to allow Mr. Don Dorsey to locate the well. Mr. Dorsey informed me prior to this inspection that he attempted to locate the casing but could not find it.
