0605921124 2017-07-19 Production Wells No Violations This inspection was conducted in regards to a complaint received from the landowner Mr. Seth McKean. Mr. McKean was concerned that the tanks did not have the proper identification at the tanks regarding the oil haulers tank numbers. Mr. McKean was advised that the Division of Oil and Gas Resource Management does not regulate tank shipping numbers. The well and tank battery was inspected. Identification was not observed at the well. The tank battery consists of (2) 100 barrel tanks and a plastic brine tank inside an earthen dike. Proper identification was not observed at the tank battery. Per OAC 1501: 9-9-05 (10) All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank(s) as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. If multiple wells are being produced into the common tank(s) each well-head shall be identified as to owner, permit number where available, well number and lease name. Time of inspection: 10:45 AM. 0648409651 2011-11-14 Production Wells Violations Noted FOLLOW UP TO NOV # -1677261878 The Dvision had received a complaint alleging no royalty payment (civil issue) and failure to submit production records for Griffith lease. Therefore, an inspection of this wellsite was conducted. The pumper, Mr. Barry Dugan, was present for the inspection. The well was equipped and capable of production. Division records show no reported production for this lease since 1999. The well pumps into a separate tank battery from Permit Nos. 4816 and 5134. The tank battery location contained a bare area extending from earthen dike and measuring approximately 120 (length) x 10. This area contained stressed vegetation such as, multiple dead trees and would extend to a width of approximately 40. Mr. Dugan states this is old and existed before he started pumping a couple months ago. High Range Quantabs placed in fluid inside dike exceeded the maximum unit of 10. Note: 8.8 units equal 6,765 ppn (mg/L)Cl-. A second Quantab along the bare path at 66 showed 8.8 units. Note: Test stopped after 8.8 units. The walls along the front of earthen dike have eroded and has reached maximum fluid capacity. The identification at the tank was not complete and will require Permit Number, Well Number, Well Name, Company Name(TJs Oil & Gas Inc.), Emergency Contact Number and County. At 12:38 pm, a call was placed to the well owner and no answer. Central Office and Management will be informed of failure to submit statements of production. The well owner shall: 1) Remove all contaminated material from inside to include fluid and outside the dike. 2) Provide complete identification at tank battery. 3) Build front wall for earthen dike. Failure to comply will result in further enforcement action.
0746604744 2013-12-09 Production Wells No Violations This inspection was conducted a compliance notice follow-up. The well was equipped with a pumping unit and motor. The polish rod shows signs of recent production. The tank battery consists of (2) 100 barrel tanks. The pollution and contamination issue at the tank battery has been resolved. Clean up was documented under p# 5134 (Griffith #4) although the Griffith #4 produces into a different tank battery located at N 40.23857 -82.23309. Violations have been resolved.

1210i35102 2001-03-01 Production Wells No Violations 100 Bbl. Tank overflowed apprx. 3 - 4 Bbl crude oil. Oil traveled down road ditch and down hollow. ( see sketch) . OEPA notified , Mike Dalton present. Oil being soaked and cleaned up. Inspector Baldwin issued NOV. for cleanup and to relocate tank minimum 50 ft. from road and to properly dike. Terry Ramsey crew 3 men on cleanup.Terry Ramsey is the pumper NOV issued to him and will mail operator a copy.

1345812883 2012-08-23 Production Wells No Violations A follow-up inspection conducted with Mr. Brandon Mitchell revealed non-compliance. The well owner has now re-located the tank battery and constructed a new earthen dike. The soil(s) did not appear to have been removed from the location. Therefore, the soil was field tested with distilled water and confirmed unacceptable Cl- level. While at the location, a voicemail message was left for Mr. Rodney Adams, the well owner, at 11:29 AM, on his cell phone requesting a return call. No return call was received on 8/23/2012.
