0078306929 2016-12-27 Production Wells Violations Noted Mr. Carl Bevard left a voicemail on 12/26/2016 notifying me of a spill incident that ocurred at the tank battery on 12/25/2016. I then contacted Emergency Operations manager Trisha Cartmell who responded to the spill on 12/26/2016. I arrived to tank battery location at 10:00 AM on 12/27/2016. Ohio EPA OSC Rikki Knerr was on location when I arrived. Emergency Operations Manager Scott King, regional manager Trisha Cartmell, and Emergency Coordinator Nathan Lane were present for clean-up operations. Mr. Bevard was doing the clean-up along with a contractor he hired. Mr. Bevard alleged that he believes someone may have been stealing oil from his tank and didnt shut a valve. This allowed oil to spill over a brine storage tank and enter a stream approximately 80 yards away. The oil was contained in the stream at a bridge near the Franklin Township garage. The oil was removed with absorbent pads and a vac truck was not needed. A roll off box was delivered and lined with plastic to properly dispose of contaminated soil. During clean-up operations oil was discovered in a ditch upgradient from the tank battery on the same side of the road. The oil was traced to a spring behind the house located at 10962 Flint Ridge Rd. This was treated as a separate incident and an investigation was started to determine where that source of oil was coming from.
0247468665 2017-01-13 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. Mr. Bevard has removed contaminated soil associated with his tank battery release. Clean-up operations have been completed on the tank battery release. During clean-up operations Mr. Bevard was advised that the brine tank would need to be set above ground instead of buried 3/4 in the ground and a dike would need to be built around the tank. Mr. Bevard agreed to do so and will complete it when weather permits. An investigation is still ongoing and is being treated as a separate incident. Time of inspection: 12:15 PM.

Those present for the inspection were Southwest Region Supervisor Dan Goins, Southwest Region District Supervisor Eric Conrad, Environmental Program Manager Kyle Tharp, Hydrologist Jake Glascock, Emergency Operations and Response Manager Scott King, and Emergency Response Regional Manager Trisha Cartmell. No violations during inspection. Mr. Brad Wells is the grandson of the homeowner and lives at this residence. Mr. Wells was not home during the inspection. The spring was not observed due to dogs in the fenced in area of the spring. Time of inspection: 11:30 AM.

The tank battery now consists of (2) 100 barrel tanks and (1) horizontal brine tank. The horizontal brine tank was still set 3/4 in the ground.
