0232559397 2011-07-08 Production Wells Violations Noted The Division received a complaint alleging non-production from this well. An inspection revealed that the well was idle at the time of inspection. The well was equipped with a pumping unit, engine, belts however, the valves were closed at the wellhead. Records show 142 bbls of oil for 2009. Verified no reported production for 2010 with Central Office. The well is currently out of production due to flowline repair, which had been allegedly damaged from logging on property. Identification was not observed at the well or tank battery. The shared tank battery consisted of (2) 100 bbl tanks, separator, drip and meter. A GPS reading was obtained for the well and tank battery locations. On 5/5/2011 and 7/6/2011, the well owner was verbally instructed to plug or produce wells on this lease prior to the issuance of this Compliance Notice. The well owner shall plug or produce this well. If said well is placed back into production, identification is required for well and tank battery(shared) locations. Provide identification at the tank battery to show permit number, lease name, well number, county, owner name, and emergency contact number. For multiple wells produced into common tank(s),the well shall also be identified as to owner name, permit number, well number, and lease name.

1747675125 2013-05-31 Plug / Plug Back No Violations Gene Matheny was preparing to plug this well. The well had 2.88 tubing on a packer. Gene unset the packer and pulled tubing. The tubing was being ran back into the well to plug with cement and could only get to approximately 2,300 feet. It was determined that the 4.5 casing parted in half as Gene pulled the tubing and packer. A bit was attached to the tubing in order to mill through the casing to get to total depth. Gene milled on the casing for approximately 5 hours and made about 12 inches.
