Salt water tank has a 2 pipe that breaches the dike wall, allowing fluids to flow unobstructed on to the ground surface. I Took a field test with a Quantab (high range) and it had a reading of 4.4 that translates in to 1,549 ppm (mg/L) Chlorides. Well over the 250 ppm standard that is allowed. I called the following phone number (740) 763-4671 at 9:53 A.M. and directed the owner to remove the pipe and that a written NOV is forthcoming. WEATER CONDITIONS: Overcast, hazy and 75 degrees.

The well location is also clean. The pump jack was running and oil was flowing into the tanks. There is proper identification displayed at the tank battery.

During the inspection, it was noted that the AD remains disconnected. The tank battery consisted of (2) 100 bbl tanks and a 50 bbl tank for saltwater storage. Instructed the well owner to cleanup pollution and contamination around the wellhead from stuffing box leak.
