1340910396 2012-06-13 Drill / Deepen / Reopen No Violations I arrived at location at approximately 10:30 am. Mike Wheeler was ready to cement long string casing during inspection. Kevin Slaughter was on location for Mike Wheeler. Total depth was at 2957 feet. Casing was set at 2950 feet.
1374191318 2013-07-18 Preliminary Restoration Violations Noted This inspection was conducted as part of a preliminary restoration status check. The well was equipped with a pumping unit and motor. A dike has been built around the well. It was pumping during inspection. This well pumps into a shared tank battery that consists of (3) 100 barrel tanks inside an earthen dike. The inspection revealed that the pit used during drilling operations has not been closed. A Quantab test strip was used in the pit water and resulted in a reading of >2000 ppm for chlorides. A 1 hose was laying inside the pit while the other end of the hose was laying within a few feet of a pond approximately 50 feet from the pit. ORC 1509.072 (A) requires that in a non-urban area pits used during drilling shall be closed by the owner within 2 months after the date upon which the drilling of a well is completed. A message was left for Mr. Wheeler on his cell phone to contact the inspector immediately. Mr. Wheeler called at 4:57 pm and was informed that the pit needs to be closed immediately. Mr. Wheeler will have until 7/22/2013 to have the pits closed and identification posted. Preliminary restoration does not meet ORC 1509.072 requirements. Photos were taken for documentation purposes.
