1375573559 2016-02-05 Administrative Inspection No Violations A phone call was placed to Dale E. McDonald (Jeremy McDonald) at 2:33 PM and was advised to monitor fluid level in pit. It was suggested that water be pumped out to keep fluid at a safe level. Jeremy stated that they would do that and would prevent pit from overflowing. Jeremy informed me that drilling operations are ongoing. The well has been drilled but they are waiting to shoot the well. Once the well is shot it will be cleaned out.
1386157754 2017-05-08 Preliminary Restoration Violations Noted This inspection was conducted as a status check. The drilling rig was set up on the well during the inspection. The well was not in production at the time of inspection. The well was shut-in with a swage and valve. The inspection revealed that the drilling pit has not been closed as of this inspection. The pit will need to be closed within 10 days. According to ORC 1509.072 (A) the drilling pit shall be closed within two months after the date upon which the drilling of a well is completed. 1423450791 2015-12-17 Casing String Witnessed No Violations I witnessed the cementing of the 5.5 surface casing for Dale E. McDonald on Jeremy McDonalds cable tool rig. Jeremy and Mike McDonald were on location. Cemenco was contracted to cement casing. Cemenco broke circulation and good cement returns were observed. Cement dropped approximately 3 feet. A basket was not used. The ticket was received. Time of inspection: 2:30 PM.
1454935267 2016-02-04 Drill / Deepen / Reopen No Violations Inspect drilling operations. No personnel on location during inspection. The rig was still on location. The 5.5 had a swage and valve on it. During the inspection oil was observed on the pit water and just outside of the pit. It appears that a recent large rainfall event has allowed oil/water to breach pit bank. The amount of oil outside of the dike would approximately be less than a gallon. The pit water was field tested with a Quantab titrator. The field test revealed a 5.0 reading which equals approximately 224 PPM Chlorides. This is below EPA Secondary Drinking Water standards of 250 PPM Chlorides. The pit was properly lined during inspection. Photos were taken of pit. The well owner will be notified and advised to prevent fluids from escaping pit. The inspection also revealed cement has dropped on the backside of the 5.5 further than the three feet mentioned in the previous inspection. Cement will need to be added and filled to surface.
