0203783788 2017-12-27 Production Wells Violations Noted I received a phone call today at 10:38 AM from landowner Mrs. Rhonda Beck regarding this well. Mrs. Beck was inquiring about the status of the well. I told Mrs. Beck that the well is not in compliance and that a follow-up inspection had just been conducted recently. Mrs. Beck said she wants the company to do something with the well and not just let it set idle. Mrs. Beck also expressed concern regarding a 100 barrel tank not in use near the well that is leaning and considers it to be a safety hazard. I told Mrs. Beck that I would contact the company and have them remove the tank. During my inspection I took four photos of the tank. I placed a phone call to well owner Sarah McCowan at 2:10 PM and advised her that the 100 barrel tank near the well shall be removed. Mrs. McCowan said she would speak with Mark Hardesty and have it removed. Time of inspection: 1:30 PM.
0774668500 2018-04-18 Production Wells No Violations My inspection revealed that the well has been placed into production. The well was capable of production as it was equipped with a pumping unit and gas motor. I observed identification at the well and tank battery. The well owner Sarah McCowan informed me that the landowners are interested in using the gas from the well for their building. I spoke to the landowner Mr. Ed Beck by phone and he confirmed that they indeed are interested in using the gas. I told Sarah McCowan and Mr. Ed Beck that the well is now in compliance and I will close out the compliance notice. The Edward H. Everett Company will not need to apply for a plug permit now. 0937882644 2017-03-21 Administrative Inspection No Violations I received a phone call from the new landowner Ms. Rhonda Beck (419-733-4272) at 10:04 AM regarding the Edward H. Everett Co. #7 well on the property. Ms. Beck was inquiring about the status of the well and would like for it to be produced or plugged. Ms. Beck was informed that a compliance notice was issued to the company last fall for non-production and a follow-up inspection will be conducted in the coming months.
1388096133 2017-12-18 Production Wells No Violations I conducted a follow-up inspection today. The well has not been plugged or placed into production. The landowners have recently built a metal building on the property approximately 150 feet to the east of the tank battery. Photos were taken of the tank battery and new building. 1418331792 2018-03-07 Production Wells No Violations I conducted a follow-up inspection on t:his well today. The well was not in compliance. I advised well owner Sarah McCowan on 1/11/2018 that the well shall be in compliance by April of 2018. On 1/17/2018 via email The Edward H. Everett Company requested a longer timeframe to achieve compliance. On 1/17/2018 Southwest Region Supervisor Dan Goins granted The Edward H. Everett Company an extension of August 1st, 2018 if a plugging permit is in hand by May 1st, 2018. Sarah McCowan has informed me that they are going to put a motor on the pumping unit and test the well to see what it will do. Time of inspection: 9:30 AM.
1496770631 2017-06-06 Production Wells No Violations This inspection was conducted as a compliance notice follow-up. The inspection revealed that the well has not been plugged or placed into production. The well was incapable of producing. Vegetation has overtaken the well. The well is not in compliance. Since the previous inspection the new landowner has put a camper with running water and electric near the tank battery. A drive way was also installed that the camper is sitting on. An electric service pole has been installed in the dike wall of the tank battery and is within 7 feet of a 100 barrel tank. Several photos were taken of the well, tank battery, and proximity of the camper to the tank battery. The company will be contacted regarding this inspection. Time of inspection: 1:25 PM.
1515522110 2018-01-09 Production Wells No Violations My inspection revealed that the 100 barrel tank near the well has been removed. The well is no longer in violation of ORC 1509.23. The company has also removed the brush and vegetation from the well. I advised well owner Sarah McCowan on 1/11/2018 that the well shall be in compliance by April 2018. Time of inspection: 12:45 PM.
1904178934 2016-11-16 Production Wells Violations Noted This inspection was conducted as a status check. The well was equipped with rods, tubing, and a pumping unit but no motor. The well was not capable of production. The well and pumping unit was covered in vegetation. A 100 barrel storage tank was observed approximately 75 feet away. The tank was in poor condition and was close to tipping over onto its side. The tank battery consists of (2) 100 barrel tanks inside an earthen dike. The tank battery was overgrown with vegetation and trees. Identification was not observed at the tank battery or well. Photos were taken of the well and tank battery. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509.12 (B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 1:15 PM.
