0348108866 2017-08-17 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. After completing repairs to the clutch assembly on the service rig, drilling continued at a depth of 148. Drilling stopped for the day at a depth of 198 before tripping out of the hole to change bits. While removing the last joint of 3.5 drill stem with the bit, the wedding band type slips failed allowing this joint to drop to the 198 mark. Several attempt were made to grab the lost piece of pipe with the 2.375 tubing yielded no success. The location was secured and the Contractor will attempt to retrieve the lost pipe on 8-18-17.

ON 8-17-17, While removing the last joint of 3.5 drill stem with the bit, the wedding band type slips failed allowing this joint to drop to the 198 mark. The contractor has fabricated a fishing tool using a 2.375 inch diameter sub inside of a 5.5 inch diameter casing. The casing creates a guide for the lost drillstem to help thread to the sub. This tool was successful in retrieving the lost drill stem and bit. The Contractor then began to rig up to continue drilling with a 9.75 inch button type roller cone bit on 8-19-17.
0830395632 2017-08-10 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. Field Order #6 and Change Order #2 were processed for Washover Equipment from Weatherford Tools that arrived on location on 8-9-2017. While rigging up the washover equipment the pipe stuck in the hole moved. A final attempt was made to free the pipe which was successful. All the remaining pipe was removed from the hole along with the 8.67 inch bit. Field Order # 7 & Change Order #3 were drafted for the price adjustments for the unused washover equipment. A 3.75 diameter cable tool was ran down the well to determine if the hole was caved in where the pipe was stuck. This tool went to an estimated depth of 575 before hitting an obstruction. A logging truck is scheduled for 8-11-17 to log the well before continuing drilling.

Field Order # 7 & Change Order #3 were processed for the price adjustments for the unused washover equipment. A logging truck from Timco Wireline Services was on location for a CCL and Caliper Log on the well showing clean hole to 565 feet except for a washout area at 140 feet. After logging the well, drilling continued with a 9.75 inch roller cone bit. While drilling through a harder area around 145 feet, the clutch on the rig started experiencing issues. All pipe was removed from the hole and preparations were made to repair the clutch.

After completing repairs to the transmission on the rig, the Contractor continued cleanout and reaming of the well with a 9.75 inch button type roller cone bit at a depth of 340 feet. This continued to a depth of 460 feet at which cleanout was stopped to prepare the well for setting 7 inch diameter Surface Casing per the Orphan Well Contract. All drill pipe was removed from the hole and the location was secured for the evening.

The cement was displaced with approximately three (3) barrels of fresh water, shut in, and left to set overnight. On July 11th, the Contractor started laying the #304 stone for the access lane and clearing of the location around the well. On July 12th the access lane to the wooded area was completed and the Contractor finished clearing the area around the well. Several loads of the larger #57 and #4 stone for the well location was also delivered and will continue into tomorrow. A Safety Meeting with the Village of Ottawa Hills, Ottawa Hills Police & Fire, Lucas County EMA, Hydrocarbon Well Service, and DOGRM Emergency Response Division is scheduled for Thursday July 13th at 10:00 am.
1499960438 2017-07-13 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The stone access lane was completed along with most of the stone around the work area. A Safety Meeting with the Mark Dixon of the Village of Ottawa Hills, Ottawa Hills Police Chief John Wenzlick , Lucas County EMA Director Patricia Mooney, Hydrocarbon Well Services Steve Jones, and DOGRM Emergency Response Divisions Terry Whitehair and Fred Romeo was conducted at 10:00 am. After excavating around the well to a depth of 5 feet, the Contractor used 4 inch by 4 inch wood blocks to create a cellar around the well.
1500307220 2017-07-17 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. Cleanout of the existing 10 (ten) inch casing started at 8:30 am. Due to the presence of a zone of lost circulation at 20 (twenty) feet, circulation could no longer be maintained. A total of 40 (forty) barrels of water were lost in the well before circulation was lost completely. The Contractor is directed to proceed with Contingency Line Item number 33: Lost Circulation Material. The Contractor is bringing 10 (ten) sacks of bentonite gel and six (6) sacks each of Cello-Flake and Hole-Plug. The quantities used will be documented by the Contractor and the Division of Oil & Gas Resources Management Inspector. Cleanout will continue on Tuesday, July 18th.

Two (2) sacks of Hole-Plug Bentonite was dumped down the well from the surface, along with a sack of Cellophane Flakes. At that time a leak was discovered in the radiator of the pump. The radiator was removed but repairs will take multiple days. Work was stopped until repairs to the pump are completed.

On 7-20-17 the contractor returned to finish repairs to the mud pump then started cleanout of the well. Circulation was lost again so the decision was made to attempt to seal the lost circulation zone with cement and Lost Circulation Material. A total of 15 sacks of cement, 1 bag of cellophane, and one bag of gel were mixed and pumped down the well through the existing ten (10) inch casing. The cement was displaced with approximately four (4) barrels of fresh water, shut in, and left to set overnight.

As part of a continued effort to seal the lost circulation zone between 20 and 40 feet, a fifth squeeze job with cement and Lost Circulation Material was conducted. A total of 20 sacks of cement and one bag of cellophane were mixed and pumped down the well through the existing ten (10) inch casing. The cement was observed circulating up the back side of the 10 inch casing and around the lined cellar around the well. The cement was displaced with approximately one half (.5) barrels of fresh water, shut in, and left to set overnight. There was no further observed contamination of the in the Ottawa River during the pumping of the cement.
1501507199 2017-07-26 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. A fifth squeeze job with cement and Lost Circulation Material to seal the lost circulation zone between 20 and 40 feet was conducted on 7-25-2017. On 7-26-2017 the cement from the previous day was observed at 3 (three) feet below grade inside the ten (10) inch casing. A 9.875 inch roller cone bit on 3.5 inch diameter drill stem was used to drill to a depth of 38 (thirty-eight) feet. There was no further observed contamination of the in the Ottawa River during cleanout of the well and good circulation was maintained. All operations were stopped at this point to allow the crew to go home for an extended weekend. The portable toilet was moved inside the orange safety fence and the location was secured. The Kubota Track-Hoe was loaded and removed from the location. Operations are scheduled to resume on Monday July 31st.
1501613657 2017-08-01 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The Contractor mixed 8 bags of gel in 40 (forty) barrels of water before starting drilling at a depth of 38 (thirty eight) feet with a 9.875 inch roller cone bit on a 34 (thirty four) feet long, 3.5 inch diameter drill stem. After clearing the drill stem, drilling continued by adding 2.375 inch diameter upset tubing. Drilling stopped at approximately 100 feet and a second batch of gel was mixed using an additional 9 bags. The well was circulated again for ten minutes before stopping operations for the day. There was an increase in the sheen in the Ottawa River during the cleanout of the well that slowed throughout the day as depth was made. This sheen was observed by Mark Thompson of the Village of Ottawa Hills. It was his opinion that the increased contamination was actually less than was observed in the past before any plugging operation began on the well.
1501786329 2017-08-02 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The Contractor starting drilling at a depth of 102 feet with a 9.875 inch roller cone bit on a 34 (thirty four) feet long, 3.5 inch diameter drill stem and then 2.375 inch diameter upset tubing. Drilling stopped at approximately 140 feet after it was determined there were too many cuttings in the half tank. A larger half tank was requested and will be on location on 8-3-17 along with the D6 Dozer.
1502125629 2017-08-07 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. On 8-5-17, a second squeeze job to seal the lost circulation around the 10 inch casing was conducted. A total of 10 sacks of cement were mixed and pumped down the well through the existing ten 10 inch casing. On 8-6-17, a third squeeze was conducted. A total of 5 sacks of cement, with no Lost Circulation Material were mixed and pumped down the well through the existing 10 inch casing. The cement was displaced with approximately one barrel of fresh water, shut in, and left to set overnight. On Monday, 8-7-17, drilling resumed with a 8.375 inch diameter roller cone bit. The progress was fast down to 155 and then slowed considerably. Good circulation was maintained throughout the day. At a depth of 226 feet, drilling was stopped and the crew started to trip out all pipe to examine the bit. The first two joints came out with no resistance but the third joint bound up at around 160 feet. The pipe would no longer rotate or go up or down. A total of 45,000 pounds of pull were put on the pipe with no success. There were no issues maintaining circulation while attempting to free the pipe. There was no further observed contamination of the in the Ottawa River during the cleanout of the well.
1503328171 2017-08-08 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. A total of two more attempts were made to free the 160 of pipe still frozen in the hole with no success. The pipe will still not rotate or go up or down but easy circulation can be maintained. Field Order # 4 was drafted and processed for a cost proposal from Hydrocarbon Well Service to remove the stuck pipe. Field Order #6 and Change Order #2 were drafted for Washover Equipment from Weatherford Tools scheduled to be on location on 8-9-2017.
1507229574 2017-08-21 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The Contractor continued cleanout and reaming of the well with a 9.75 inch button type roller cone bit at a depth of 290 feet. This continued to a depth of 340 feet before the transmission on the rig became disabled and required replacement. Plugging operations were shut down until further notice while the repairs to the transmission can be completed. The well was secured and will be monitored until plugging operations continue.

The 7 inch diameter Surface casing was ran to a depth of 448 feet on 9-8-2017 and the hole was circulated. The hole was circulated again before pumping cement. The contractor used 96 sacks of Class A cement to cement the 7 inch casing back to surface. The cement did start to drop back immediately after displacement and will be remediated on 9-10-2017. All cementing equipment was cleaned and the location was secured for the evening.

1675707988 2017-07-31 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The Contractor is not on location at the time of the inspection. The well is still secure with no sign of leaking gas or fluids at the wellhead or in the cellar. The water level in the Ottawa River was down another 5 inches on the bank and at less than one foot in the middle. A small amount of oil was evident in the booms around the area of the creek where the initial contamination was discovered during cleanout. This oil has accumulated over the last four days and will be cleaned with pads when the Contractor arrives. The Contractor contacted me at 11:55 stating that due to delays, they will not be on location until 8-1-2017.
1720803654 2017-08-04 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The Contractor starting drilling at a depth of 2 feet with a 8.375 inch diameter roller cone bit on a 34 (thirty four) feet long, 3.5 inch diameter drill stem and then 2.375 inch diameter upset tubing. Drilling stopped at approximately 155 feet when circulation was seen on the backside of the 10 inch conductor casing. A squeeze job to seal the lost circulation around the 10 inch casing was conducted. A total of 17 sacks of cement were mixed and pumped down the well through the existing ten 10 inch casing using water mixed with bentonite gel from the return tank. The load was displaced with 3 barrels of water and shut in for the night. A second squeeze job is scheduled for 8-5-2017.
1796748729 2017-09-11 Orphan No Violations On location for the Lucas #1 Orphan Well Plugging Project by Hydrocarbon Well Service. The cleanout of the well continued with a 5.5 inch diameter roller cone bit inside the 7 inch casing set on 9-9 & 9-10 at a depth of 400 feet. The cleanout continued with no issues to a depth of 611. At that depth the pipe was lifted back up to clean that section of hole again due to an increase in cutting that appear to be in the wellbore. When the pipe was sent back down, cutting were again encountered at 578. This process was repeated six times with the same result each time. The decision was made by Orphan Well Program Manager Gene Chini to plug the well at 611 and not risk the hole collapsing around the tools. Plugging of the well will begin on 9-12-2017. All drill pipe was removed from the hole and the location was secured for the evening.

On 7-24-17, a fourth squeeze job to seal the lost circulation zone between 20 and 40 feet with cement and Lost Circulation Material was conducted. A total of 10 sacks of cement, with no Lost Circulation Material were mixed and pumped down the well through the existing ten (10) inch casing. The cement was displaced with approximately three (3) barrels of fresh water, shut in, and left to set overnight. There was no further observed contamination of the in the Ottawa River during the pumping of the cement.
