0996262942 2018-01-30 Production Wells No Violations The purpose of my inspection was to document the status of identification at well #2-1544. At the time of my inspection identification was made legible at the production tank. No further action is necessary.
1012144507 2010-08-23 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead. I continued to walk around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
1457708673 2018-01-16 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1544. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. Access to the well and tank battery was snow covered. Identification posted at the well head was illegible. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. No leaks were observed. Staining consistent with a legacy operation was present. A snow covered dike was present around the production equipment. Equipment within the dike consisted of one (1) vertical separator and one (1) 210 bbl production tank. The separator remained in good repair and contained a current 31 day production chart. The production tank remained in good repair and contained illegible identification. Valves were closed and plugged. I will contact lease operator Jeremy Kohler to update the identification within ten (10) days.
1741950190 2012-12-06 Production Wells No Violations On 12/06/2012, I performed a status check on the above well site. The tank battery is equipped with one 210 barrel tank, one in-ground tank and one vertical separator. The dike is in good condition. The well head has a pumping unit and electric motor. Identification signs are posted. No violations noted at this time.
