0027745522 | 2015-04-14 | Production Wells | No Violations | I performed a status check on the above location. There is two culverts installed in the lease road. In 2014 Enervest installed a 12 culvert at the south end of the lease road, at this time and they are running water. The well is equipped with one 100 barrel separator and one 35 barrel water tank and one vertical separator. The well is equipped with 1.5 tubing. ID signs are posted. The dike is insufficient to contain a release at this site. I will notify Enervest to correct the dike. |

1321551570 | 2011-11-14 | Production Wells | No Violations | Received complaint from Ken Grisdale (330-533-3284) regarding unidentified well and lease road causing poor drainage on property. The well is properly identified with ID sign on separator. No leaks or spills around well head or tank battery. Lease road in good condition. The portion of property in question exhibits wetland characteristics, such as standing water, and wetland vegetation, i.e.. Typha spp., Phragmites spp. There is one 12 culvert installed under lease road in wetland area. Well head, tank battery, and lease road within compliance. No violations at this time. |
1400789341 | 2014-05-22 | Production Wells | No Violations
Inspection of the well site revealed a plunger lift system well and a tank battery. The tank battery contained a 100BBl production tank that had paint peeling off of it and rust spots, a poly tank and a mechanical separator were also present in the battery. The dike around the tank battery is weathered and insufficient to contain a release from the production tank. The tank battery is up slope from the stream. The following actions need to be taken at the well site: 1. Dike needs rebuilt around the tank battery 2. The production tank and equipment needs to be scrapped and painted 3. Install an additional culvert at the south end of the riparian zone crossing were water is backing up. | |
1402925630 | 2014-05-08 | Production Wells | No Violations | On 5/7/2014, I received a phone call from Ken Grisdale stating that the water from this location is backing up on his pasture. On 5/8/2014, I meet him at the location. He would like for Enervest to open the ditch along the lease road and also open up the wet land area so the water will flow away from his property. I told him that I would talk to my supervisor about what could be done to help with this problem. There is two culverts installed in the lease road at this time and they are running water. The well is equipped with one 100 barrel separator and one 35 barrel water tank and one vertical separator. The well is equipped with 1.5 tubing. ID signs are posted. The dike is insufficient to contain a release at this site. I will call Enervest about the dike. |

1409745230 | 2014-06-13 | Production Wells | No Violations | I performed a follow up inspection on the above location. Northeast Region Supervisor Tom Hill had contacted Enervest on 5/22/2014, about installing a culvert at the south end of the lease road to help relieve the water back up on the west side of the lease road, painting the 100 barrel production tank and repairing the dike to hold the capacity of the 100 barrel tank. At this time no work has been done. I called Larry and left him a message to contact me about a time frame when the work would began. |
