0183638599 2012-02-28 Production Wells No Violations I conducted a status check on this site and found that the well is equipped with a pumping unit and connects to a tank battery that consists of three 210 barrel stock tanks, two 100 barrel brine storage tanks, four separators, and an earthen dike. The tank battery is shared with permits 1277, 1278, 1406, 2544, 2545, and 3048. Proper identification was posted and I found no leaks or spills at the well head or tank battery. No violations at this time.
1247685211 2009-07-15 Production Wells No Violations I walked around the tank battery and the wellhead and found that there were no violations at this time.

1554131413 2019-04-01 Production Wells No Violations Follow-up to a tank explosion and fire that occurred on 3/31/2019. I was on location with Paul Shaw (Dorfman) and landowner Robert Martig to inspect the location. The tank involved in the incident is a 210 barrel tank used for the production of the of this well and the Martig #s 2, 3, & 5 (PN#s 2544, 2545, & 3048). The battery also contains one (1) 210 barrel tank for the Lerch #s 1 & 2 (PN#s 1277 & 1278), one (1) 210 barrel tank for the Hinton #2 (PN# 1406), one (1) 100 barrel tank, and a 50 barrel poly tank that were unaffected by the incident. The east end of the battery contains one (1) heated, horizontal separator and three (3) vertical separators with associated meter runs. The heated separator is 60 from the tanks, meets the required set back, and the heater is not being utilized. There is currently no trigger for the tank fire and explosion. The top of the tank blew off and landed 360 to the southeast in an agricultural field. The tank vent stack hit and damaged the siding on a house 1000 to the southeast. Paul has spoken with the landowner and they are going to repair the siding. The tank bottom expanded, but remained intact. The only oil released was from the dump line where it detached from the tank and affected an area 6 X 6 inside secondary containment and has been covered with Peatsorb. I spoke with Paul and he informed me that the water has been taken off and that there is four (4) feet of oil in the tank that will be shipped today. The tank will be removed and replaced and the contaminated material will be disposed of properly. There was no violation issued at this time.
1933282297 2005-12-16 Production Wells No Violations The tank battery is shared with permits: 1278, 1277, 1406, 2544 and 2545 and consists of 4-210 bbl storage tanks, 3-100 bbl salt water tanks, 3-separators and a dike.
