0899143279 2012-04-04 UrbanProduction Wells Violations Noted I received an email on April 2, 2012 from Michael Novak (mjnovaksr@yahoo.com) in regards to the condition of a tank battery near his home. I conducted a status check on this site and found that the well head is equipped with a plunger lift system. There were no leaks or spills but there was no identification on the well head. The tank battery is shared with permit 2672 and consisted of two 100 barrel stock tanks, one 100 barrel brine storage tank, two separators and an earthen dike. The tanks were rusted and the wood fence around the tank battery was in poor repair. Many of the wooden slats were falling off, the door was off the hinges and the paint was peeling off the fence. I contacted Dave Rice of D&L Energy to report the issues and to request that they be corrected as soon as possible.
1305449210 2012-05-11 Production Wells No Violations I conducted a follow up inspection on this site after issuing a notice of violation for general safety and failure to legibly identify well. On the day of my inspection I found that the board on board fence around the tank battery had been repaired, the stock tanks were freshly painted and there was proper identification posted at the well head. The issues have been resolved. No violations at this time.
1407939545 2014-08-12 UrbanProduction Wells Violations Noted On 8/7/2014, I received an e-mail from Jennifer Gingras from the Division of Oil & Gas UIC Section that was forwarded to her by Beth Wilson from Public Information. The e-mail, originally from Michael Novak stated that there is a strong natural gas odor at this location and the fence around the tank battery needs repaired. When I arrived on location there was a strong odor of gas at that time. I performed my inspection of the tank battery. There are three 100 barrel tanks and two horizontal separators. This tank battery is also shared with 3409922672 Brothers #1. As I walked the area I observed that the dike had eroded away causing the fence to become unstable and that the weather has destroyed some of the vertical slats on the fence. I noticed that the lids on the 100 barrel tanks would not completely close causing natural gas to escape from the tanks. The tanks are vented to the atmosphere. At the west side of the dike there is a open cut into the dike letting water drain from the tank pad onto private property. There is an area of dead vegetation outside the dike with dimensions of 80 X 20. I tested the water inside the tank battery with Quantab. The chlorides readings were 6562 ppm. There is a dwelling within 100 of the tank battery. I called Dave Rice, Field Supervisor for D&L Energy, and instructed him to shut the wells in until the repairs are made. The tank battery fence needs replaced or repaired. The tanks needs to be replaced or fitted with lids that will not permit gas to escape to the atmosphere and have charcoal filters and spark arrestors installed. Contaminated soil from the tank battery and lawn area needs to be removed and disposed of at an approved site. The dike needs to be repaired to contain the volume of the largest tank that is installed inside the tank battery.
1429711841 2015-04-17 UrbanProduction Wells No Violations On 8/12/2014 I issued a Compliance Notice on the above location #1407939545. Up on my inspection. The contaminated soil has been removed and disposed of. The fence has been removed and a new one installed. There has been two new 100 barrel tanks with sealed lids installed. There has been a charcoal filter installed on the vent lines from the tanks. ID signs are posted. At this time there is no noted violations. This Compliance Notice can be closed.
