0071334020 | 2012-04-20 | Production Wells | No Violations | On 4/20/2012, I completed a production well inspection status check for DNR Inspector Chris Grimm. This well shares a lease road with permit numbers 2-2856 and 2-2831. My inspection found the well head was a plunger unit (rabbit) that was in good condition. The tank battery contained one 100 barrel oil tank with the proper identification posted, 50 barrel brine tank, and a separator. I found no visible spills or leaks. The lease road was in good condition and there are no violations at this time. |
0117390002 | 2007-07-16 | Preliminary Restoration | No Violations | I performed a status check for perliminary restoration. GPSd well. Pit liner in one pit was down some 17 feet along the pit. Called Dick Heldman to tell him about it. He said that Wade Pol was aware of it and that this afternoon a truck would suck the pits out and pull liner back up. Checked on later and the pit liner was up were it should be. I found no other violations at this time. |
0142734051 | 2007-07-11 | Drill / Deepen / Reopen | No Violations | Performed a rig inspection. GPSs well. Dpeth was 4382. Pit liners in good shape. I found no violations at this time. |
0573712891 | 2007-08-02 | Preliminary Restoration | No Violations | I performed a preliminary restoration check. Pits are closed. Debris is gone. Drilling equipment is gone. Location is leveled out. This well has not passed preliminary restoration. |
0631167098 | 2007-07-08 | Drill / Deepen / Reopen | No Violations | Reports from the drilling rig indicated that the top of cement from the surface job was over 200. The decision was made with Chesapeake to run a cement bond log in order to accurately locate the top of cement. On 7/8, Key Energy ran a cement bond log that indicated that the top of cement was at 79. Apparently the rig hands misjudged the top with the tag line. I told Chesapeake that they could grout the 79 after the drilling has been completed. |
0849318197 | 2007-10-11 | Preliminary Restoration | No Violations | I performed a preliminary restoration status check. GPSd both the well and tank battery. ID sign was posted. Drilling equipment was gone. Debris is gone. Vegetation is growing. This well does PASS PRELIMINARY RESTORATION. |

1184782428 | 2007-07-18 | Drill / Deepen / Reopen | No Violations | Met with Wade Pol and Dick Haldeman at the wellhead. They dumped 3 yards of 9#/sack of sand and cement down the annulus of the 8 5/8 casing. The annulus had a siphon 3/4 pvc tube for the water to exist out. Cement top was at 79. Had plenty of cement. Job complete. |
1549214397 | 2010-09-14 | Production Wells | No Violations | This status check was requested by Jay Cheslock. I performed a field inspection and walked around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time. |
